Clear communication after a visit helps patients understand what comes next and helps practices keep care plans moving.

For independent practices, simple follow-up routines such as confirming next steps, documenting key instructions, and closing callback loops can strengthen continuity and reduce confusion for patients and care teams alike.

PACN supports the kind of practice infrastructure and collaboration that helps independent physicians stay focused on superior, patient-centered care.

Your most complex patients deserve more than reactive care.
As a PACN-ACO member, you have access to Complex and Chronic Care Management resources designed to help you deliver proactive, coordinated care for patients managing multiple chronic conditions. That means better outcomes, fewer gaps in care, and real support between office visits.

This is the infrastructure independent physicians need but rarely get on their own. With PACN-ACO, it's already built in.

Productive payer conversations start with organized practice information.

Before performance, quality, or contract discussions, independent practices benefit from reviewing the basics: current quality measure progress, patient follow-up workflows, documentation patterns, access trends, and operational questions that need clarification.

PACN helps independent physicians navigate an increasingly complex healthcare environment with practical support, shared knowledge, and a network built around preserving strong, community-based care.

Independent practice is a choice worth protecting.

The PACN-ACO was built by physicians, for physicians. That means the structure, the support, and the decisions all reflect what matters most to you: staying independent while delivering the highest quality care to your patients.

Value-based care doesn't have to mean giving up control. With PACN-ACO, it never has.

Value-based care depends on more than reporting at the end of the year. It starts with the everyday systems that help practices understand patient needs, close care gaps, and act on information in a timely way.

For independent practices, data readiness can mean clearer follow-up lists, more consistent documentation, and better visibility into quality priorities before they become urgent.

PACN supports practices with shared resources and a collaborative network designed to help physicians focus on superior, patient-centered care.

This week, Dr. John Pagan, Chair & CEO of the PA Clinical Network, had the privilege of speaking before a room of independent physicians at the Independent Physician Summit, on the invitation of the Pennsylvania Medical Society.

We want to extend a sincere thank you to the Pennsylvania Medical Society for the invitation and for its leadership, vision, and unwavering support of the clinical community, and to its dedicated staff, whose behind-the-scenes work makes moments like these possible.

A special thank you as well to Dr. Arvind Cavale for his exemplary contributions and tireless advocacy on behalf of independent physicians across Pennsylvania.

Conversations like those at this conference matter because independent medicine matters. We're proud to stand alongside partners who share the conviction that physicians can stay independent and thrive, and we're grateful for every chance to advance that work together.

Thank you.

Independence and high-performing, coordinated care aren't mutually exclusive.

As a physician-led, clinically integrated network sponsored by the Pennsylvania Medical Society, PACN exists to give independent practices the infrastructure, support, and shared accountability that have long been treated as the exclusive territory of larger health systems, without asking physicians to give up what makes their practice theirs.

Recently, at the Independent Physician Summit, Dr. Pagan walked through what that has meant for member physicians on the ground: the resources to succeed in value-based care, a partner that understands the realities of running an independent practice, and a network built around the conviction that physicians practice better when they aren't doing it alone.

The takeaway for the independent physicians in the room was a simple one - you don't have to choose between staying independent and thriving.

If that's a conversation you'd like to have for your own practice, we'd welcome it. Reach out to learn more about PACN - [email protected].

This Fourth of July, we're celebrating more than the nation's 250th birthday, we're celebrating the independent physicians of Pennsylvania who embody the same spirit of self-determination that founded this country 250 years ago, right here in our Commonwealth at the Pennsylvania Medical Summit.

Every day, PA's independent physicians choose to stay rooted in their communities, build lasting relationships with patients, and keep the physician-patient relationship at the center of care; even as consolidation, regulatory burden, and financial pressure make that choice harder every year.

At PA Clinical Network, we've seen firsthand that independence and collaboration aren't opposites - they're complementary. By sharing resources, measuring quality, and advancing value-based care together, physician-led organizations are proving they can lead healthcare forward while preserving the autonomy that lets doctors do what's best for their patients.

Thank you to everyone who joined us at the Independent Physician Summit in Harrisburg, and those who join us in spirit. Your commitment to independent medicine is shaping a stronger future for our patients and the medical profession.

Wishing our physicians, staff, and patients a safe, joyful Fourth of July.

Revenue cycle performance depends on more than billing alone. Clear documentation, timely follow-up, consistent workflows, and strong communication between clinical and administrative teams all play a role in helping independent practices stay financially and operationally steady.

For practice leaders, small process improvements can make daily work more manageable. Reviewing open claims, identifying recurring denials, confirming patient information, and creating clear handoffs can help teams reduce avoidable friction while keeping attention on patient care.

PACN supports independent practices with collaboration, shared resources, and practical infrastructure designed to help them navigate an increasingly complex healthcare environment.
