Your nervous system enters the room before your words do.

I’ve named this pattern in my own work for years, and it doesn’t have a name in most helping professions: Impaired Presence — the quiet erosion of relational quality that happens when the person providing care, teaching, or leadership is operating from a dysregulated nervous system.

It’s not self-care. It’s ethics. The person on the other side of that power differential — the patient, the student, the employee — carries the impact of our internal state whether we’re aware of it or not.

We’ve been told the wrong story about why helping professionals leave.

“The patients were too much.” “The students were too hard.” “The clients burned them out.”

That’s the convenient version. The truer one: they left because no one held them. Leadership wasn’t regulated enough to hold the people doing the holding.

This isn’t a critique of any one leader — it’s a pattern most systems haven’t been taught to see.
