Outreach Community Health Centers celebrates National Health Center Week! We proudly celebrate the impact of community health centers and the patients, partners, and neighborhoods we serve every day.

At Outreach Community Health Centers, we've been honored to continue expanding access to quality healthcare across Milwaukee. Here's just a glimpse of what we've accomplished together this year:

• Opened and expanded access through our Racine Outreach Community Health Center.

• Launched our Mobile Health Clinic in collaboration with Ascension Wisconsin, bringing healthcare directly into Milwaukee neighborhoods.

• Continued important conversations through on WNOV, connecting our community with trusted health information.

• Partnered with organizations across Southeast Wisconsin to provide health screenings, education, and resources at community events.

• Welcomed thousands of patients for primary care, behavioral health, preventive services, and whole-person care—regardless of their ability to pay.

AND FINALLY...we're bringing our community together on Saturday, August 8 for the 2026 Community Health & Resource Fair, connecting families with healthcare and local organizations.

Yesterday marked a milestone for healthcare access in Milwaukee as Outreach Community Health Centers proudly celebrated the ribbon cutting of our new Mobile Health Unit clinic at Ebenezer Church Milwaukee, made possible through our collaboration with Ascension Wisconsin.

This innovative mobile clinic will bring high-quality primary care directly into neighborhoods, helping remove barriers such as transportation, cost, and access. Together, we're expanding primary care, preventive services, and health resources beyond the walls of a clinic and into the communities we serve.

Thank you to our dedicated team, community partners, elected officials, and everyone who joined us to celebrate this exciting step forward. This is more than a mobile clinic. JOIN THE Outreach Community Health Centers TEAM: Under the direction of the Director of Quality, the Population Health Specialist uses population health strategies to address the preventive and chronic care needs of the patient population. This includes analysis of patient health data to identify care gaps, outreach to patients to help them obtain needed services, and collaborating with clinic staff to ensure care coordination.
