We are looking to fill the position of contract Targeted Case Manager.
What is Targeted Case Management?
•Needs Assessment: Evaluate the client’s medical, social, and psychological needs to identify their unique strengths and vulnerabilities.
Service Coordination: Link clients with appropriate community providers and secure funding for required therapies, housing, or vocational programs.
Care Planning: Develop and maintain highly individualized service plans outlining specific goals, objectives, and timelines.
Monitoring & Advocacy: Regularly follow up with clients to assess their health and safety, adjusting plans as their situation changes and advocating for their rights across service systems.
Crisis Intervention: Provide immediate assistance, coping skill development, and support during stressful situations or mental health emergencies.
