At our monthly Journal Club, our group of local doctors (all practicing in Direct Primary Care clinics in the area) recently reviewed a major new clinical practice guideline on adult sinusitis. This comprehensive update was published in July 2025 in Otolaryngologyโ€“Head and Neck Surgery, the official journal of the American Academy of Otolaryngologyโ€“Head and Neck Surgery (AAO-HNSF) โ€“ written by ENT specialists to guide better care.

Many โ€œsinus infectionsโ€ are actually viral (like a bad cold), and they get better on their own. The new guideline strongly recommends watchful waiting for most uncomplicated cases โ€“ often waiting up to 10 days (or reassessing in 3โ€“5 days once diagnosed) before considering antibiotics. Jumping straight to antibiotics just because someone reports sinus-like symptoms isnโ€™t always the right move anymore.
Instead, the focus is on supportive, anti-inflammatory care that helps you feel better faster and safer.

This approach reduces unnecessary antibiotics, lowers side effects, helps fight antibiotic resistance, and still gets great results for patients.

The Cure Chronicles by Ernesto Matos Perez

**"My labs were normal... so why do I still feel like I'm running on 5% battery?"**

She sat down and apologized before we even started.

*"I know you're going to tell me it's stress. Everyone else has."*

She wasn't asking for a miracle.

She just wanted to know why climbing a flight of stairs felt like hiking a mountain.

Why her brain felt wrapped in fog.

Why she needed a nap after grocery shopping.

Why she didn't feel like herself anymore.

Sometimes the first set of blood tests really is reassuring.

But "normal" doesn't always mean "everything has been evaluated."

Medicine is often about putting together patterns, not chasing a single abnormal number.

Persistent fatigue can sometimes overlap with:

โ€ข Iron deficiencyโ€”even before anemia develops.
โ€ข Thyroid problems.
โ€ข Low vitamin B12 or vitamin D.
โ€ข Sleep apnea that disrupts sleep without you realizing it.
โ€ข Insulin resistance and blood sugar swings.
โ€ข Deconditioning after illness.
โ€ข Dysautonomia, where the nervous system struggles to regulate heart rate and blood pressure.
โ€ข Medication side effects.
โ€ข Chronic inflammation or other medical conditions.

The goal isn't to assume the worst.

The goal is to ask better questions.

Your body is constantly sending signals. Fatigue isn't always something to "push through." Sometimes it's your body's way of saying that one of its systems needs attention.

Every person deserves to be heard, examined carefully, and evaluated as an individual.

If you've been told, "Everything looks normal," but you still don't feel like yourself, it may be worth taking another thoughtful look.

Sometimes the diagnosis isn't found in a single lab value.

Sometimes it's found by connecting the dots.
