For many people, the question "What if my health changed?" feels too heavy or too far away. You can make it lighter by asking it this way instead: "What matters most to me right now?"
That could be:
β
My independence β being able to make my own choices
β
My relationships β staying connected to family, friends, or community
β
My routines β morning coffee, music, prayer, exercise, cooking, hobbies
β
My comfort β feeling safe, calm, respected, and not overwhelmed
So instead of asking "What if my health changes?" ask: "What do I love about my life right now that I would want people to understand?"
This is how advance care planning starts β not with fear, but with clarity.
Guide2Care's 12-Week Health Future Plan walks you through everything:
β
Clarify your health care wishes
β
Organize your health information
β
Choose a healthcare proxy
β
Prepare advance directives & key paperwork
β
Have the conversations your family may one day need By the end, you won't just have documents β you'll have clarity, direction, and a plan your loved ones can actually follow.
