At Aventura, we believe strong partnerships are at the heart of exceptional patient care. This past week, we had the pleasure of co-hosting an appreciation dinner for the incredible case managers from Mercy Youngstown, alongside our wonderful colleagues at Harmony Hospice.
It was a meaningful opportunity to come together, strengthen relationships, and recognize the dedication of the case managers who work tirelessly to help ensure patients and their families experience thoughtful, seamless transitions of care.
We are grateful for the opportunity to be a trusted partner in that journey and for the collaboration that allows us to better serve our patients and families.
