Why You Must Stop Ozempic® and Other GLP-1 Medications Before Surgery

If you’re taking medications such as Ozempic®, Wegovy®, Mounjaro®, Zepbound®, Trulicity®, or other GLP-1 receptor agonists, it’s very important to let your surgeon know before your procedure.

These medications work by slowing the emptying of your stomach, which helps you feel full longer and aids in weight loss. While this is beneficial for weight management, it can become a safety concern during surgery.

Even if you’ve followed the fasting instructions and haven’t eaten for several hours, food may still remain in your stomach because of the medication. During anesthesia or sedation, the body’s normal protective reflexes are reduced. If stomach contents come back up into the throat, they can be inhaled into the lungs (called aspiration), which can lead to serious complications such as pneumonia, difficulty breathing, or, in rare cases, life-threatening lung injury.

For this reason, your surgeon and anesthesiologist may ask you to stop your GLP-1 medication before surgery. The exact timing depends on the specific medication you’re taking, your medical history, and the type of procedure you’re having. Some patients may also be asked to follow a liquid diet before surgery or undergo additional evaluation if there is concern that the stomach has not emptied normally.
