Sudden facial droop is an emergency-room visit first, every time — not "wait and see." Stroke has to be ruled out before anything else. Bell's palsy is a diagnosis of exclusion: it's confirmed by ruling out the more dangerous causes, not by a single test. Is your eye twitching when you smile actually normal — or is it synkinesis?

After facial paralysis recovers, some patients notice something strange: their eyelids contract when they chew, or the corner of their mouth pulls when they blink. That's called synkinesis. It happens when regenerating facial nerve fibers miswire and a signal meant for one muscle also fires another.

It's common after Bell's palsy and other facial nerve injuries. To a large extent it is treatable. However, it's important to understand that there is currently no 100% reliable "cure" for synkinesis. But surgical innovations and strategies exist to improve facial spasticity and symmetry. This is a tough problem, but one that I tackle routinely.
