💙 HAPPY FRIDAY FROM PATIENT CARE COORDINATION! 💙

August is a month to celebrate wellness, appreciation, empowerment, inclusion, and independence — values that are at the heart of everything we do at PCC.

Every day, our mission is bigger than coordinating care. We help individuals and families navigate services, overcome barriers, and move toward greater independence, dignity, and quality of life. ♿💜

✨ This August, we celebrate:
💙 Wellness and well-being
💙 Caregivers who make a difference every day
💙 Disability awareness, inclusion, and empowerment
💙 The strength of individuals living life on their own terms
💙 Communities working together to create opportunity for everyone

At PCC, we believe that independence isn’t just a goal — it’s a journey worth supporting.

As we head into the weekend, take a moment to celebrate someone who inspires you through their strength, compassion, or dedication. Tag them below and let them know! 👇💙

Have a beautiful Friday and an amazing weekend from all of us at Patient Care Coordination!

At Patient Care Coordination (PCC), protecting the health, safety, and independence of the individuals we serve goes beyond service coordination—it means sharing timely information that empowers our participants, caregivers, and communities to make informed decisions every day.

This July, we’re highlighting simple but important wellness reminders to help you stay safe during the summer months. Whether it’s staying hydrated, protecting yourself from extreme heat, or reaching out when you need support, small steps today can make a lasting difference tomorrow.

Your health. Your independence. Our commitment. 💙

Smoke from wildfires burning in Canada is affecting air quality across parts of Pennsylvania. Poor air quality can be especially harmful for older adults, children, and individuals living with asthma, COPD, heart disease, or other chronic conditions.

Protect yourself and those you care for:

Stay indoors when air quality is unhealthy, keep windows and doors closed, use a HEPA air purifier when available, limit strenuous outdoor activity, and wear a well-fitted N95 mask when going outside is necessary. Check local conditions regularly through AirNow.gov and follow Pennsylvania public health updates.

Patient Care Coordination Inc. is actively monitoring conditions and remains committed to participant safety, continuity of care, timely communication, and assistance during environmental health events. Participants and caregivers who need support or notice changes in their health should contact their care team. Seek immediate medical attention for severe difficulty breathing, chest pain, confusion, or other serious symptoms.

💙💚 PREVENTION IS YOUR POWER! 💚💙

Your health is your greatest investment—and the best time to take care of it is before problems begin.

At Patient Care Coordination (PCC), we believe prevention leads to healthier lives, greater independence, and peace of mind. Staying on top of routine preventive care can help detect health concerns early, when they’re often the most treatable.

✔️ Annual Preventive Visit
✔️ Colorectal Cancer Screening
✔️ Breast Cancer Screening (Mammogram)
✔️ Blood Pressure Monitoring
✔️ Annual Diabetic Eye Exam
✔️ HbA1c Screening for Diabetes Management

✨ Healthy habits matter too!
🥗 Eat nutritious foods
🚶 Stay physically active
💧 Stay hydrated
😴 Get enough sleep
💊 Take medications as prescribed
🧠 Prioritize your mental health
🚭 Avoid to***co and limit alcohol

💙 Medicare vs. Medicaid… Do You Know the Difference? 💚

Many people use the terms Medicare and Medicaid interchangeably—but they are very different programs, and understanding the difference can help you or a loved one receive the right care and benefits.

🏥 Medicare
✔️ Federal health insurance
✔️ Primarily for adults age 65+
✔️ Also covers certain younger individuals with disabilities and specific medical conditions

🩺 Medicaid
✔️ Joint federal and state health insurance program
✔️ Designed for individuals who meet income and eligibility requirements
✔️ May provide access to Home & Community-Based Services (HCBS), long-term services and supports, and more

✨ Did You Know?
Some individuals qualify for both Medicare and Medicaid, often called Dual Eligibility, which can significantly reduce healthcare costs and expand available benefits.

At Patient Care Coordination, we’re committed to helping individuals and families understand their healthcare options, navigate complex systems, and connect with the services they deserve.

📞 Have questions about your benefits or need help navigating the healthcare system?

We’re here to help.

💜 June is Alzheimer’s & Brain Awareness Month

Every memory tells a story. Every act of kindness makes a difference.

At Patient Care Coordination, we’re proud to support individuals and families through compassionate, person-centered care that promotes dignity, independence, and quality of life.

This month, take a moment to:
🧠 Learn the warning signs.
💜 Support caregivers.
🤝 Show compassion.
🌿 Encourage brain health.
🏡 Help build stronger communities.

Together, we can make sure no one faces Alzheimer’s or other forms of dementia alone.

Learn more about Alzheimer’s disease, caregiver resources, and brain health by visiting trusted organizations like the Alzheimer’s Association. Small actions and greater awareness can have a lasting impact on our communities. 💜

This Memorial Day, Patient Care Coordination Inc. honors and remembers the brave men and women who made the ultimate sacrifice in service to our country. 🇺🇸

Today, we reflect on the values of compassion, dignity, resilience, and community — principles that continue to guide the work we do every day alongside participants, families, providers, and partners throughout the Community HealthChoices program.

At PCC, we remain committed to supporting participant choice, independence, health, safety, and person-centered care while recognizing the courage and sacrifices that allow our communities to thrive.

As we gather with loved ones this holiday, we encourage everyone to take a moment to remember those who served and those who never made it home.

From our PCC family to yours, we wish you a safe, meaningful, and peaceful Memorial Day weekend.

Where compassion leads, quality care follows. 🇺🇸
