Deprojection (making the nose smaller on side profile) is one of the most commonly requested changes in rhinoplasty. It’s also one the most difficult maneuvers in rhinoplasty.

Our wonderful patient presented requesting a smaller and straighter nose with a natural profile.

The challenges in this case:
- Thin skin
- Tension nose deformity
- Dorsal hump
- Deviated to the right
- Overprojected tip and nostrils

The solutions:
✅ Burr hump reduction
✅ Piezo bone narrowing
✅ Septoplasty with bony reset
✅ Bilateral ANSA banner grafts
✅ Medial crural overlay (nostril deprojection)
✅ Tip onlay graft
✅ Cadaver rib graft used
✅ 8 grafts used

The one year result is a smaller, straighter and more feminine nose that she loves.

As always, swipe 👈 for every angle.
