Mid-State Health Center Mid-State Health Center is a non-profit community health center whose mission is to provide sound primary care to all, regardless of ability to pay.

Mid-State Health Center delivers primary care and supportive services at both its Bristol (100 Robie Road, 603-744-6200) and Plymouth (101 Boulder Point Drive, 603-536-4000) offices offering medical services, dental care, behavioral health counseling, outpatient addiction treatment, and care management to all ages.
