Otago is one of the most heavily studied fall prevention programs that exist - and that's the honest reason we recommend it.

✔ The original trial, published in the British Medical Journal in 1997, followed 233 women aged 80 and older living at home. Over one year, there were 88 falls in the group doing the exercises, compared with 152 in the group that didn't.
✔ A 2010 review in Age and Ageing pooled seven trials and roughly 1,500 older adults. It found Otago cut the rate of falls by close to a third.
✔ A 2021 analysis in PLOS One pooled 12 trials and found Otago improved measured balance and reduced fear of falling - which matters just as much as the numbers. Being steadier is one thing. Trusting yourself enough to go to the store alone is the thing that actually changes your week.

Here's the honest part, because you deserve the whole picture: in that 2010 review, only about a third of participants were still exercising consistently a year in. Follow-through, not difficulty, is the real challenge with a program like this.

What does a fall prevention program that's actually backed by research look like day to day?

Not a lecture. Not a pamphlet. The Otago Exercise Program is 17 strength and balance exercises plus a walking plan, done three times a week, about 30 minutes at a time - built to be done at home, often next to a kitchen counter or a sturdy chair.

Here's roughly what's in it:

✔ Leg strengthening for the muscles that actually catch you when you stumble
✔ Sit-to-stand practice, because getting out of a chair safely is a real-world skill
✔ Balance work that gets progressively harder - feet together, heel-to-toe, one leg, then walking backwards and sideways
✔ A walking plan built around what you can do today, not what a 40-year-old can do
✔ Progression - the plan gets adjusted upward as you get stronger, which is the part people almost always skip when they try to do this alone

It starts with a full assessment, then guided visits over the first eight weeks while the plan gets tailored to you, then you carry it yourself with regular check-ins.

Most people assume that real fall prevention has to cost something - a gym membership, a trainer, ongoing visits.

For a lot of the older adults we see here in Salem, that isn't true.

There's a fall prevention program with decades of research behind it, and for many qualifying seniors, it costs nothing. We deliver it here at HWY Physical Therapy through our partnership with Northwest Senior and Disability Services (NWSDS).

A lot of people don't realize they're entitled to see their plan of care in writing.

So here's what that actually means at HWY Physical Therapy.

A plan of care is not a guess, and it's not one-size-fits-all. It spells out your specific goals, along with how often and how long you'll need to come in to reach them - built around your history, your strength and balance today, and any other health conditions in the mix.

It's also not static. As you progress, Dr. Raj reassesses and adjusts - fewer visits as you get stronger, more focus on the exercises that are moving the needle, less on the ones that aren't.

And because home-exercise adherence is the biggest factor you actually control, your plan will always include what to do between visits - not just what happens in the clinic.

You're entitled to ask for this in writing at any point in your care. A good plan of care welcomes the question.

Here's a mistake we see more than any other at HWY Physical Therapy: stopping once the pain eases, before the strength underneath has actually caught up.

It makes sense in the moment. The pain is gone, life is busy, and PT feels like something you can check off the list. But pain easing and strength rebuilding are two different timelines - and research shows the benefits fade when the exercise stops too soon.

So what should you actually expect along the way? A good plan of care has real milestones built in:

✔ By visit 2-3, you should understand what's actually driving your symptoms
✔ By week 4, you should see real, measurable change
✔ By week 8, genuine strength change - and often, visits start tapering as you take over more of the work yourself

If you're somewhere in the middle of your own plan right now, this is a good week to ask: "Where am I against these milestones?" It's a fair question, and your PT should have a real answer.

It's one of the first questions almost every new patient asks us at HWY Physical Therapy - and understandably so. Nobody wants to commit to something open-ended.

Here's what we tell people: anyone who gives you an exact number over the phone, before ever examining you, is guessing.

Two people can walk in with the exact same diagnosis and need very different amounts of care. It depends on how long the problem has existed, how much strength or balance has already been lost, what other health conditions are in the mix - and the biggest factor you actually control, how consistently you do your exercises between visits.

There's also a myth worth clearing up: needing more visits doesn't mean your case is worse. It usually just means your body needs more time to rebuild what it lost.

Here's something about arthritis that catches a lot of people off guard.

When a joint hurts, the natural instinct is to protect it - move it less, sit more, wait for it to settle down. That makes complete sense. But with arthritis, resting too much usually makes things worse:

✔ The muscles around the joint get weaker, so it loses the support it depends on
✔ The joint stiffens the less it moves, which makes the next movement hurt more
✔ Doing less often means a little weight gain, which adds load to sore knees and hips
✔ And a smaller, more careful life quietly chips away at confidence, too

So a cycle sets in: it hurts, so you move less, so it gets weaker and stiffer, so it hurts more.

The encouraging part? That same cycle runs in reverse - when you get the right kind of movement, in the right amount, guided the right way. Joints are meant to move. The right movement is often the medicine, not the thing to avoid.

We hear that almost every week - usually with a little shrug that comes right along with it.

And we understand why. Arthritis is common, it comes on slowly, and the stiffness can start to feel like it's just part of the furniture.

But here's the honest truth: for most people, arthritis is not a life sentence of pain.

You may not be able to erase it entirely. But how much it hurts, how stiff you feel, and how much it limits your day - those are things we can absolutely work on.
