Edward Dieguez Jr MD Dr. Edward Dieguez Jr., an Anesthesiologist/Interventional Pain Specialist, and IOF Fellow. He has been in practice in Saint Augustine, Florida since 1988.

When is Platelet Rich Plasma (PRP) indicated and when is the more advanced option of Bone Marrow Aspirate Concentrate (BMAC) indicated

1. PRP is used for less severe Orthopedic and Musculoskeletal conditions.
• Early to moderate osteoarthritis of the knee, hip or shoulder
• Chronic tendinitis of rotator cuff, tennis elbow, wrist.
• Mild ligament injuries and sports related soft tissues injury
• Very small tendon tears.

2. BMAC is used for more severe Orthopedic and Musculoskeletal issues.
• Moderate to advance arthritis with cartilage loss
• More advanced tendon tears or stubborn ligament injuries
• Cartilage damage that limits mobility
• More advance sports injury that needs a more regenerative approach
Both therapies above may be combined in certain cases as some research suggests PRP and BMAC together may offer synergistic healing benefits.

PROS AND CONS OF BOTH PLATELET RICH PLASMA (PRP) AND BONE MARROW ASPIRATIONS CONCENTRATE (BMAC)

1. PRP Advantages and Limitations:
• Advantages:
a. It is a minimally invasive procedure and is quick.
b. Works well for mild arthritis, mild tendinitis and soft
tissue injuries.
c. Your own blood is used so has low risk
d. Short recovery time.

• Limitations:
a. It may not be effective for severe and advanced joint
damage.
b. Multiple injections are sometimes needed.
c. Results vary between patients.

2. BMAC Advantages and Limitations:
• Advantages:
a. Contains stem cells and may regenerate
cartilage/bone
b. Helpful for more advance arthritis or injuries that have
not healed with other modalities.
c. It may provide longer lasting results than PRP in some
cases.
d. It can be used for healinf after surgey.

• Limitations:
a. Slightly more invasive since bone marrow has to be
extracted.
b. Recovery may take a bit longer than PRP
c. Not always necessary for mild conditions

WHY IS FLUOROSCOPY/ULTRASOUND GUIDANCE SO VITAL WHEN UNDERGOING INJECTIONS FOR PAIN?

As you can see in the picture bellow it take precision to make sure the tip of the needle end up in the right place after it passes the skin. It make injection effective and most importantly less risky because it helps you avoid hitting sensitive structures that can cause disastrous complications. In this picture you can see the needle inside the lumbar face joint to treat arthritic lumbar facet joints. Dont increase the risk and decrease the efficiency of your injections by having them done blindly with no fluoroscopy or ultrasound guidance. I call it the riskiest guess work possible!

ARE YOU LOOKING FOR PLATELET RICH PLASMA (PRP) FOR YOUR PAINFUL JOINTS, SPINE CONDITIONS OR SPORTS INJURIES ?

Remember not all PRP are the same! You want FDA clear, doble centrifuge highly concentrated, high dose PRP. And also depending on the area to be treated, your doctor has to draw from 60 to 120 ml of blood from you.

Additionally, for the injection to be performed correctly, it cannot be done blindly and the local anesthetic to be used, it cannot be chondrotoxic! You need to use Fluoroscopy or Ultrasound during the procedure so you can be sure that the tip of the needle ends up in the target area that needs to be treated!

When the procedure is done in this fashion by an experience physician specialized in these procedures, you have the best chance of it working. Success rate varies between 70 and 80 %. And also keep in mind that from the time of injection you need to wait around three months to get the full effect. At that time if you have not improved at all, you need to then consider surgery. And if your pain has improved 50 or 60% you have the option of getting a second PRP injection.

This is all done in our office under local anesthesia in our fully equipped sterile procedure room. You walk in and you walk out in about an hour!

So, be wise and if you find out that in the place you are considering to have the procedure done, they only draw a 10 ml tube of blood or that the injection is done blindly without Fluoroscopy or Ultrasound, and they use Lidocaine as local anesthetic, you better run away and don’t waste your time or your money.
