Turning Excess Tissue Into an Opportunity for Reconstruction

This lovely woman in her 40s came to me after massive weight loss, hoping to remove excess skin from her upper arms. But during our consultation, we discovered an opportunity to address more than just excess skin.

She also had a history of hidradenitis suppurativa, a chronic inflammatory condition that had left significant scarring in both armpits (axillae), affecting the quality of her skin and soft tissue.

Together, we developed a surgical plan that addressed multiple concerns in one reconstructive approach.

Rather than discarding the excess tissue removed from her upper arms, we repurposed it to reconstruct the scarred tissue in both axillae and enhance the shape and volume of her breasts through autologous breast augmentation.

✨ No breast implants were needed. We used her own tissue to restore, reshape, and reconstruct.

This case is a powerful example of how reconstructive surgery goes beyond removing excess skin. By thinking creatively about how tissue can be used, we can address multiple concerns, restore form and function, and help patients move closer to their personal goals.

Sometimes, the tissue we remove can become the very tissue that helps rebuild. ❤️

Reconstructive Microsurgery: Personalized Solutions Through Innovative Tissue Transfer

This case is a great example of how reconstructive microsurgeons can repurpose areas of excess tissue from one part of the body to reconstruct defects in another.

Using advanced microsurgical techniques, a patient’s own tissue can be carefully transferred along with its blood supply to restore form and function, while simultaneously addressing areas where tissue may be in excess.

The goal is reconstruction tailored to the individual patient. By understanding the unique anatomy, tissue characteristics, and reconstructive needs of each patient, microsurgeons can develop personalized solutions that optimize both functional and aesthetic outcomes.

Clinical Case Spotlight: Complex Elbow Reconstruction 🩺

This patient in his 30s sustained an open fracture of the right olecranon (elbow), classified as a Gustilo-Anderson Type IIIB injury, with significant soft tissue loss.

Although the orthopedic team successfully repaired the fracture, a substantial open wound remained, requiring advanced reconstructive surgery to provide durable soft tissue coverage.

Using vascularized tissue from the upper arm, we were able to reconstruct the defect and provide healthy, well-perfused tissue to cover the exposed structures around the elbow.

Why does this matter? Severe traumatic injuries often require more than bone repair alone. Adequate soft tissue coverage is essential to protect underlying structures, support wound healing, and help preserve long-term function.

This case highlights the importance of a multidisciplinary approach to complex extremity trauma, combining orthopedic fixation with advanced reconstructive techniques to help patients recover from devastating injuries.

I was recently fortunate to attend a course on craniofacial peripheral nerve surgery for migraine headaches at the Migraine Surgery Symposium during Plastic Surgery The Meeting 2026 in Houston.

It was a privilege to learn from leaders in the field and exchange ideas with international colleagues who share a commitment to helping patients with difficult-to-treat headache disorders.

I look forward to continuing to expand the peripheral nerve and reconstructive options I can offer my patients.

This patient in his 60’s was diagnosed with a large squamous cell carcinoma of the scalp that had slowly grown over several years. He was seen in my office in consultation for both removal of this tumor and reconstruction. At the time of surgery his skin cancer was found to extend into the underlying skull bone. Removing the cancer required removal of the involved scalp and bone.

Because this left a complex defect with exposed skull, To rebuild the area, we performed a free flap reconstruction—transferring healthy, well-vascularized tissue from another part of the body and reconnecting its blood vessels using microsurgery. A skin graft alone would not provide enough healthy, vascularized tissue to adequately cover and protect the exposed bone—especially when additional cancer treatment such as radiation may be needed. Free-flap reconstruction provides durable, vascularized coverage and can help restore the scalp while supporting healing and allowing for additional cancer treatment when needed.

We also neurotized the flap by connecting nerves from the flap to nerves in the scalp using microsurgery in order to provide protective sensation to the scalp in the future. He has since gone on to recover well and has returned to his busy farming life!
