# Testosterone for Women: An Important Hormone That Is Often Overlooked

When people hear the word testosterone, they often think of men. However, women naturally produce testosterone as well. Although women have much lower levels than men, testosterone contributes to sexual desire, arousal, and sexual satisfaction.

For some women, changes occurring during and after menopause can be accompanied by a noticeable loss of sexual interest. This can affect confidence, emotional well-being, and intimate relationships. In properly selected patients, carefully managed testosterone therapy may be worth considering.

# # What Symptoms Might Lead to an Evaluation?

Women frequently seek a hormone evaluation because they are experiencing:

* Persistent loss of sexual desire
* Reduced sexual arousal or satisfaction
* Less motivation for intimacy
* Vaginal discomfort or painful in*******se
* Fatigue, poor sleep, or mood changes
* A general sense that they no longer feel like themselves

These symptoms do not automatically mean that a woman needs testosterone. Fatigue, mood changes, and low sexual desire can have many causes, including estrogen deficiency, thyroid disease, anemia, medications, chronic stress, depression, poor sleep, vaginal dryness, relationship concerns, and other health conditions.

A comprehensive evaluation is therefore more useful than simply treating a laboratory number.

# # When Is Testosterone Therapy Evidence-Based?

The strongest scientific evidence supports testosterone therapy for carefully selected postmenopausal women with **hypoactive sexual desire disorder**, commonly called HSDD.

HSDD refers to a persistent loss of sexual desire that causes personal distress and cannot be better explained by another medical condition, medication, psychological concern, or relationship issue.

International guidelines report that appropriately dosed testosterone can produce a meaningful, although generally moderate, improvement in sexual desire, arousal, or**sm, pleasure, and sexual satisfaction for some postmenopausal women with HSDD.

Testosterone is not currently established as a general treatment for weight loss, brain fog, osteoporosis, disease prevention, or anti-aging. It should be prescribed to address an appropriate clinical concern—not simply because a laboratory value appears low.

# # Why Blood Testing Alone Is Not Enough

There is no single testosterone level that diagnoses a “testosterone deficiency syndrome” in women or predicts who will benefit from treatment.

Blood testing is still valuable. It helps establish a baseline, identify unexpectedly high levels, guide safe dosing, and prevent treatment from raising testosterone above the normal female physiologic range. The decision to treat, however, should be based on a woman’s symptoms, medical history, examination, contributing factors, and treatment goals—not on one number alone.

# # What Are the Possible Side Effects?

When testosterone is used conservatively and maintained within the normal female range, it is generally well tolerated in appropriately selected patients. Possible side effects include:

* Acne or oily skin
* Increased facial or body hair
* Scalp hair thinning
* Mood changes
* An excessive rise in testosterone levels

Voice deepening and cl****al enlargement are uncommon at physiologic doses but may become irreversible if excessive doses are used. Proper dosing, follow-up, and laboratory monitoring are essential.

Long-term safety information remains limited, particularly regarding breast and cardiovascular outcomes. Each woman’s personal and family history should be reviewed before treatment.

# # Is Testosterone Approved for Women?

There is currently no testosterone product specifically approved by the FDA for women in the United States. When prescribed for women, testosterone is therefore used “off-label.” Off-label prescribing is common in medicine, but patients should understand the expected benefits, limitations, alternatives, and potential risks before beginning treatment.

# # Hormone Therapy Should Be Individualized

Low desire during menopause is rarely explained by only one hormone. Estrogen deficiency, vaginal dryness, painful in*******se, medications, sleep, stress, emotional health, and relationship factors may all contribute.

For some women, addressing estrogen or vaginal symptoms may be the first priority. For others who continue to experience distressing low desire after contributing factors have been addressed, a carefully monitored trial of testosterone may be reasonable.
