Year-end has a way of arriving quickly. Before attention shifts completely to 2027, this is a good time to look closely at what is happening inside your revenue cycle.

Start with five areas:

1. Aging A/R. What is moving, and what has stopped moving behind the scenes?
2. Recurring denials. Which issues keep coming back, even after individual claims are resolved?
3. Underpayments. Are you consistently receiving what you expected? More or less?
4. Payer trends. Where are requirements, behavior or turnaround times changing? What other trends can you spot?
5. Authorization breakdowns. Are preventable issues kicking off before the claim is ever submitted?

The goal isn’t to chase every number before December 31. It’s to understand where revenue is getting stuck, which problems are systemic and what deserves a plan going into the new year.

More than 15 million Americans live in a county with no general surgeons and nearly 10 million live in a county with no specialty surgeons., according to a recently-released analysis from the American College of Surgeons and the University of Carolina. That includes 434 Critical Access Hospitals in rural counties with no recorded full-time surgeon.

For rural healthcare leaders, the issue goes beyond workforce shortages. Access and financial sustainability are closely connected.

Revenue cycle can’t solve a surgeon shortage. But capturing earned reimbursement, reducing preventable revenue loss and maintaining strong financial visibility can help protect the resources rural organizations need to sustain care in their communities.

More reports don’t always mean more answers. Working a denial doesn’t mean you’ve solved the problem. And “your numbers look fine” doesn’t tell you much about what’s happening behind them.

The real treat is actionable insight., fewer repeat problems, and a revenue cycle you actually understand.

When it comes to RCM, fewer tricks and more clarity is always the best kind of treat.

Revenue cycle problems rarely get easier with time.

At 30 days, an unresolved account may look manageable.

At 90 days, the details are getting older. Follow-up has been delayed. The path to payment may be getting more complicated.

At 120 or 180+ days, what started as a small issue can become much harder to recover.

The same is true for recurring denials, payer issues and broken processes. Left unaddressed, they don’t just affect one account. They can continue creating new ones.

That’s why having a partner who knows where revenue is getting stuck, understands why and can address problems early helps keep small issues from becoming expensive ones. Find out more at inlandrcm.com.

Working denials matters so much, but preventing denials might matter even more.

Your RCM partner should be helping you understand what’s driving repeat denials and where changes can stop them before they happen.

Lower patient volume doesn’t mean fewer complexities. Rural hospitals and Critical Access Hospitals operate in a unique financial environment, navigating specialized reimbursement methodologies, diverse payer requirements, limited staffing resources and a wide range of services, often with far less room for error.

So when teams are lean, even a seemingly small disruption can have an outsized impact.

That’s why rural RCM requires more than general revenue cycle knowledge. It requires an understanding of how rural healthcare works in reality, not just in theory. The right processes, expertise and support can help protect revenue while giving internal teams more room to focus on the work that matters most.

Quality healthcare is built on thousands of details done well, each and every day.

During National Healthcare Quality Week, we’re recognizing the people across rural healthcare who work to make care safer, stronger, and better for our communities.

Quality reaches beyond the bedside, too. Accurate information, thoughtful processes, timely billing, and a healthy revenue cycle all help create the stability rural healthcare orgs need to keep delivering exceptional care.

Cheers to the teams behind the scenes and on the front lines who keep raising the bar.
