We help older adults stay strong, independent, and safe at home with one-on-one PT & OT. We like to think of ourselves as an outpatient physical therapy clinic that provides treatment in the home setting. Our physical therapist spend on average 60 minutes one-on-one time with each patient visit. Treatment is provided by a licensed physical therapist, no assistants or aides.

Falls don’t have to be a normal part of getting older.

Changes in balance, strength, walking ability, or confidence can increase fall risk and make everyday activities feel less safe. Likewise, the home environment can impact an individuals risk for falls.

At Smart Rehab, fall prevention is a core clinical focus of our in-home outpatient therapy. Our licensed therapists work one-on-one with older adults to identify barriers to safe mobility and build practical strategies that support greater confidence and independence at home.

Therapy may include:

One of the biggest questions we hear from patients, families, and healthcare professionals is: “What are the options when someone still needs therapy, but no longer qualifies for home health?”

Smart Rehab is a locally operated, therapist-owned outpatient therapy provider that delivers outpatient Physical, Occupational, and Speech Therapies directly in the patient’s home.

Unlike traditional home health, patients do not need to be homebound, and a physician referral is not required for patients and families to get started. Services can be billed with traditional Medicare under Medicare Part B, and most supplemental plans are accepted. Private-pay options are available as well.
