Your training zones are only part of the story. 📊

Most metabolic testing can tell you your VO₂ max, identify your zones, and hand you a report.

But the more important question is:

What should you actually DO with that information?

At Physician Coach, we combine VO₂ max + lactate testing to look deeper at your physiology. We analyze your LT1 and LT2, lactate response, substrate utilization, heart rate, pace or power—and importantly, where those thresholds occur relative to your VO₂ max.

Why does that matter?

Two athletes can have the exact same VO₂ max and need completely different training.

One may have a strong aerobic foundation but need more threshold and high-end aerobic work. Another may have plenty of aerobic capacity sitting there—but needs to build the aerobic efficiency to actually use it.

Then we add the piece that numbers alone can’t answer:

What are you training for?

A marathon. An ultra. A functional fitness competition. Military selection. Your first endurance event. Or simply staying fit, capable, and active for decades.

Your physiology + your goal determines the training priority.

That’s why we don’t want to simply give you zones.

We want you to leave knowing:

Where are you now?
Where is your biggest opportunity?
What should you train to improve it?

Then we re-test and see if the physiology actually changed.

TEST → UNDERSTAND → TRAIN WITH INTENTION → RE-TEST.

Because better data should lead to better training decisions.

Your Achilles and plantar fascia aren’t separate problems. They’re part of the same system. 👣

It’s not uncommon to see Achilles tendinopathy and plantar fasciopathy happening at the same time.

Why?

The Achilles tendon, heel, plantar fascia, and surrounding fascial tissues are mechanically connected. When one part of that system isn’t tolerating load well, it can change how forces are transferred through the entire foot and ankle.

That’s why treating only where it hurts—or simply putting an insert in your shoe—may not solve the problem.

🔬 Diagnostic ultrasound allows us to evaluate both the Achilles tendon and plantar fascia in real time, identify areas of thickening, degeneration, tearing, or other pathology, and determine exactly what needs to be treated.

When appropriate, treatment may include:

🩸 Ultrasound-guided PRP to target the injured tissue and stimulate a healing response

🪡 Needle tenotomy for chronic degenerative tendon tissue

🏋️ Progressive tendon-loading to rebuild the tissue’s capacity to tolerate force

🏃 A structured return-to-running/sport progression to get you back to what you love

An insert may change how your foot is loaded. It doesn’t rebuild the capacity of an injured Achilles tendon or plantar fascia.

The goal is bigger than temporarily unloading the painful area.

Identify the pathology. Treat the injured tissue. Rebuild its capacity. Return to sport.

That’s precision sports medicine, done differently.

Not all “sciatica” comes from your back.

If you’ve been dealing with pain deep in your buttock, pain that radiates down the back of your leg, or “sciatica” that just won’t go away, the problem may not be a disc or pinched nerve in your spine.

One often-overlooked diagnosis is Ischiofemoral Impingement Syndrome (IFI).

This occurs when the quadratus femoris muscle becomes compressed between the ischium and the femur, irritating the muscle and sometimes the nearby sciatic nerve.

The result?

⚡ Deep glute pain

⚡ Pain sitting or walking

⚡ Pain that radiates down the leg

⚡ Symptoms that closely mimic sciatica

The good news?

This condition can often be diagnosed right in the office using dynamic diagnostic ultrasound.

Even better, ultrasound allows us to identify the exact tissue causing your pain and guide treatment with precision.

Treatment may include:

🪡 Ultrasound-guided dry needling

💉 Corticosteroid injection to calm an irritated muscle

🩸 PRP to stimulate healing in chronic cases

🏃 A progressive rehabilitation program to restore hip mechanics and reduce compression

Sometimes the key to getting better isn’t trying another treatment...

It’s finding the right diagnosis.

If you’ve been told your MRI is “normal,” your back isn’t the source of your symptoms, or you’ve tried treatment without success, it may be time to look somewhere else.
