Since opening in 2024, Salem Family Care has been dedicated to providing compassionate, high-quality care for patients and families in the heart of downtown Salem. We’re proud to share that Pioneer Physicians Network has received the 2026 Best Overall Heart Failure Clinical Performance Award from the agilon health Network.

This recognition was presented at the annual agilon health Network Leadership Retreat and reflects the dedication of our providers, care teams, and staff in improving outcomes for patients with heart failure.

The award was based on heart failure prevalence data and hospitalization data, showing meaningful progress in reducing heart failure-related hospitalizations. Across agilon, before the clinical pathway began, 45% of patients were being diagnosed with heart failure in an inpatient or ER setting. That number has now dropped to 5%.

We are incredibly proud of the work our Pioneer team does every day to provide high-quality, proactive care for our patients.

Pioneer Physicians Network has been named among the top 1% of primary care practices in the nation by Elevance Health.

Out of more than 7,000 providers evaluated, only 45 were recognized, and just 6 in Ohio.

This recognition reflects the work happening every day across our practices: preventive care, chronic disease management, and a commitment to doing what’s right for patients.

We’re proud to share an exciting milestone for our team! 🎉

Dr. Meghan Weiner has officially become a physician owner at Pioneer Physicians Network, effective May 1, 2026.

Dr. Weiner practices internal and lifestyle medicine at Stow-Hudson Primary Care, where she focuses on whole-person care, helping patients manage chronic conditions through the six pillars of lifestyle medicine, including nutrition, exercise, and overall wellness. She also provides women’s health services, including routine GYN screenings and menopause care.

Her commitment to patient-centered care and community wellness has already made a meaningful impact, from her clinical work to initiatives like Walk with a Doc.

Throughout the month of March, our team at Pioneer Physicians Network came together to collect pet supplies, food, and cleaning products to support local animals in need. Thanks to the generosity of our staff, we were able to donate a wide range of essential items to help make a difference.
