The Age-Friendly Health Systems (AFHS) movement has advanced adoption of the 4Ms (What Matters, Medication, Mentation, and Mobility) framework, but health systems need evidence of whether or not adherence is associated with outcomes.

The authors of a new article describe an effort that adapted and implemented previously developed electronic health record (EHR)–based 4Ms measures at a large academic medical center. In a retrospective cross-sectional analysis, the authors measured encounter-level adherence to 4Ms care for patients 65 years of age and older hospitalized for more than 24 hours between December 2022 and May 2025. The authors evaluated the association of 100% encounter-level adherence to 4Ms assessments plus proportions of nursing shifts with 100% of 4Ms actions taken with length of stay (LOS) and odds of discharge to a postacute care (PAC) facility.

The study sample included 12,905 unique patients who had 20,512 inpatient encounters. Of those, 36.6% met 100% adherence to the Assessment components of the 4Ms. Only 8.4% of encounters had 100% adherence to 4Ms Assessment components plus complete adherence to 4Ms Act-On components for 25% of an encounter; 2.8% had 100% Assessment adherence plus complete adherence to 4Ms Act-On components for 50% of an encounter; and 0.4% had 100% Assessment adherence plus complete adherence to 4Ms Act-On components for 75% of an encounter. Greater 4Ms adherence was associated with lower postacute facility discharge across the first three adherence thresholds and shorter LOS across all thresholds, with LOS reductions increasing from 1.4 days at Assessment plus 25% Act-On to 2.9 days at Assessment plus 75% Act-On.

These findings suggest that EHR-based 4Ms measurement by AFHS-recognized organizations can help those systems target workflow improvements to support outcome-oriented care delivery for older adults.

Industrialization, urbanization, population growth, and lifestyle changes have collectively contributed to a growing global burden of gynecologic diseases. These factors have adversely affected women’s reproductive health and overall quality of life, with repercussions that extend to their families and wider communities.

In China, county-level public hospitals serve as the primary providers of care or first line of defense for managing these conditions, highlighting the urgent need for improved service quality. However, a systematic framework with specific quality indicators for gynecologic care remains underdeveloped both in China and internationally. Compared with obstetrics, progress in gynecologic care quality assessment has lagged, creating a gap between evidence and practice.

To close this gap, the authors established mainland China’s first validated indicator system for gynecologic care quality. National implementation across 178 hospitals demonstrates the framework’s practical utility while revealing significant quality differences across hospital tiers and regions. This standardized evaluation framework provides a practical tool for targeted quality assessment and precision improvement in primary gynecologic care.

An initiative at Jefferson Health involving 5234 provisioned clinicians and more than 283,000 ambulatory encounters has been associated with reduced time spent and effort expended in documenting patient encounters, with benefits concentrated among the most documentation-burdened clinicians, particularly advanced practice clinicians.

From January 2022 to June 2025, a cohort-based implementation model used to integrate 4Ms care delivery was associated with improvement in certain clinical outcomes, while the long-term systemwide stretch goal of 95% adherence with all 4Ms has remained unmet at 45%.

A review of 40,411 inpatient encounters at Houston Methodist among adults 70 years of age or older found that only 3% of encounters achieved higher-level compliance with 4Ms (What Matters Most, Medication, Mentation, and Mobility) care documentation; however, that subgroup was associated with shorter length of stay and reduced unplanned 30-day readmissions.
