We recently had the pleasure of connecting with the outstanding case management team at Prisma Health Parkridge Hospital in Irmo. It was such a meaningful visit — learning more about the patients they serve, the dedication they bring to their community, and the exceptional care they provide every single day.

At Attentive Angels, we are proud to partner with case managers across South Carolina to support families of all ages, from pediatrics to geriatrics. Our goal is simple: help individuals return home safely, recover with confidence, and reduce the risk of unnecessary hospital readmissions through reliable, compassionate care.

Workforce shortages. Access to care. Caregiver burnout. Aging populations. These aren't just talking points — they are the daily realities that families and providers across South Carolina are navigating right now.
