September is National PACE Month. 🦋
Throughout the month, we're sharing educational information about the Program of All-Inclusive Care for the Elderly (PACE) and answering common questions about this unique model of care.
Who is eligible for the Program of All-Inclusive Care for the Elderly?
To enroll in the Program of All-Inclusive Care for the Elderly, individuals must:
• Be age 55 or older
• Live within the service area
• Meet nursing home level of care requirements
• Be able to live safely in the community with support from the program
The Program of All-Inclusive Care for the Elderly is designed to help eligible older adults receive coordinated healthcare and supportive services while remaining in their homes and communities whenever possible.
At Blue Ridge Care, the program brings together medical care, long-term services, and support through an interdisciplinary care team focused on each participant's unique needs. 💙
