Gentell’s August 2026 Wound Insider provides a comprehensive overview of the different types of debridement and the importance of accurate documentation. Medicare Regulation F686 states that wounds should have documented assessments every 7 days, and each wound should have documentation of debridement at least once during the lifetime of the wound. It is ideal to routinely document debridement of wounds and update the level of debridement as needed with the highest level of debridement currently in progress.

Gentell’s July 2026 Wound Insider addresses tunneling and undermining—two distinct wound care terms that should not be used interchangeably. The main difference between tunneling and undermining is tunneling is unidirectional (moving in one direction), whereas undermining can occur in more than one direction. As part of the wound assessment, there are times when we also need to measure tunneling and/or undermining, in addition to measuring the length, width, and depth of the wound.
