At MyRehab Dual Diagnosis Addiction Centres, we provide structured MDT reports to referring and treating mental healthcare professionals, where the patient has given informed consent.

These reports are designed to give you a clear, clinically useful picture of the patient’s presentation, progress, risks, diagnostic impressions, medication profile and discharge planning needs.

Reports may include input from:

Case Management
Family Collateral
Medical/Doctor Intake
Nursing Intake
Therapy Notes
MDT Observations
Medication Records

We can also provide specific reports on request, including:

Diagnostic Impressions
Symptom Severity
Treatment Progress
Functional Impairment
Risk Assessment
Treatment Plan
Discharge Summary
Medical/Nursing Summary
Psychiatric Observation Summary

The goal is simple: better continuity of care between inpatient treatment and the professionals involved in the patient’s longer-term recovery.
