The importance of recognizing a stage 4 flat foot with deltoid and incompetency. Chronic medioclear space widening from a mist deltoid rupture where only the syndesmosis was repaired. Patient developed chronic and stability with progressive flat foot.

Calcaneal osteotomy with Cotton osteotomy and LARS reconstruction of deltoid to restore stability and correct the flat foot.

This patient has a very rare avulsion fracture of the posterior tuberosity of the calcaneus that was left untreated. As a result they complained of a broadened heel and aching around the Achilles as it's entire attachment had detached.

Surgery involved exposing the fragment posteriorly followed by the careful dissection of the Achilles tendon away from the fragments. You can see in one of the photos the multiple fragments that were dissected free to reduce the width of the heel.

Proceeded with a standard suture anger repair of the Achilles down to the calcaneus augmented it with a dermal patch which I find gives better creep and therefore is less stiff than the normal LARS patch I would use.

Dr Corso from Italy was also speaker on techniques to repair Achilles ruptures augmenting with the LARS graft.

Not all bunions can be managed through percutaneous or minimally invasive techniques. This patient in particular and arthritis of the 2nd TMTJ and instability of the first Ray. Therefore a Lapidus fusion of the 1st and 2nd TMTJ combined with a MIS Akin can still offer good correction of the bunion and treat the arthritis simultaneously.

Complete rupture of the tibialis anterior tendon 4 months ago. The telltale lump in 1st picture is retraction of the tendon under the extensor retinaculum. The second picture showed a defect with retraction of almost 7cm, which we will unable to bridge by a end to end repair so an allograft tibialis anterior tendon was used to bridge the defect.

Important to have good repair of the extensor retinaculum back over the tendon to prevent bow stringing. Good closure of the wound at the end.

Another ankle replacement this time in significant valgus. Good correction post op with an Inbone stem, you can see where the calcaneus was impinging against the fibula in the post op AP X-ray.

Included a deltoid reconstruction and shifted the calcaneus, better restoration of the arch on the post op lateral x-ray as well.

This time a deltoid using a LARS ligament. Two anchors are placed for repair of the deltoid initially then the LARS is placed in the medial malleolus to create a two-tailed graft.

2 tunnels placed respectively in the talus and sustentaculum to recreate the superficial and deep deltoid ligaments. The remaining anchors are then passed through the deltoid ligament and the graft tightened with the ankle in neutral.
