Meet Logan, one of our Disability Support Workers.

Logan brings experience across specialist education, aged care, and disability support, and is currently completing a Bachelor of Nursing and Bachelor of Paramedicine. He takes a person-centred, trauma-informed approach, supporting participants to build confidence, develop new skills, and achieve the goals that matter most to them.

Logan is known for his compassionate nature, attention to detail, and commitment to helping people reach their potential.

Outside of work and study, Logan enjoys drawing, painting, and expressing his creativity through art.

If your teenager is autistic and you're weighing up social skills support, it can be hard to know what actually works - there's a lot of noise out there. Meet Zac, Founder and Director of Unified Care Solutions.

As a Registered Nurse and PEERS® Certified Provider, Zac is passionate about helping neurodivergent young people build confidence, develop meaningful social connections, and achieve greater independence. His approach combines evidence-based practice with practical, strengths-focused support that helps participants apply new skills in everyday life.

Having lived experience of both ASD and ADHD, Zac understands that every person's journey is different and believes the best outcomes come from listening, working collaboratively, and focusing on what matters most to each individual.

Outside of work, Zac enjoys fishing, camping, and exploring foods from different cultures. He enjoys fishing for the experience just as much as the catch—which is fortunate, because the catch is often in short supply.

With six years of oncology nursing experience before moving to Australia and three years working in community nursing and disability support, Luisa brings both clinical expertise and genuine compassion to every participant she supports.

Watching your child want friends but struggle to build those connections can be incredibly hard.

Many neurodiverse teenagers and young adults genuinely want meaningful friendships but find the unwritten rules of social interaction confusing or difficult to navigate. That can lead to loneliness, frustration and a loss of confidence for both young people and their families.

That's why Unified Care Solutions Pty Ltd is pleased to be offering the PEERS® Social Skills Program, with separate groups for:
• Adolescents (13–17 years)
• Young Adults (18–30 years)

PEERS® is one of the world's most well-researched, evidence-based social skills programs for neurodiverse young people. It provides practical strategies that help participants build confidence, develop friendships and navigate everyday social situations in ways that feel authentic to them.
Participants learn skills such as:
• Starting and joining conversations
• Finding common interests
• Building and maintaining friendships
• Handling teasing, disagreements and conflict
• Navigating social gatherings
• Dating and relationships (Young Adult program)

A unique feature of PEERS® is that a parent, caregiver or trusted support person participates as a Social Coach. Together, you'll learn practical strategies that can be reinforced between sessions, helping new skills become part of everyday life.

Our program is delivered by PEERS® Certified Providers in a supportive, respectful environment that recognises and values neurodiversity.

Meaningful friendships, confidence in social situations, and the ability to navigate everyday interactions are important components of wellbeing, independence, and community participation. Yet many adults experience persistent challenges in these areas and are unsure where to turn for evidence-based support.

That is why Unified Care and Support (UCS) is pleased to offer places within our PEERS® for Young Adults Program.

PEERS® (Program for the Education and Enrichment of Relational Skills) is an internationally recognised, evidence-based social skills intervention developed at UCLA. Designed for motivated adults experiencing social difficulties, the program provides practical, real-world strategies for:
- Developing and maintaining friendships
- Entering and exiting conversations appropriately
- Using humour and finding common interests
- Handling disagreements and conflict
- Navigating social gatherings and get-togethers
- Building confidence in social situations

Our program follows the Unified Social Model of Care (USMC), combining:
- Therapeutic sessions delivered by a PEERS® Certified Provider
- Weekly caregiver or support person coaching to strengthen social coaching skills
- Community-based support from Disability Support Workers to facilitate the practice and generalisation of newly learned skills in everyday settings

We welcome referrals from Allied Health Professionals, Medical Practitioners, Support Coordinators, and families seeking structured, evidence-based support for young adults who would benefit from improved social competence and connection.
