Bone remodels. Fat compartments shift and descend. Skin thickness changes. Collagen and elastin production slows — but not randomly, and not all at once.

This is the part that gets skipped when ageing gets treated as an emergency instead of a process: there's a sequence to it. And the sequence matters more than most treatment plans account for. A line that shows up because of volume loss underneath it needs a completely different approach to a line that shows up because of skin thinning on the surface. Treat the wrong layer and you'll get a result that looks slightly off, even if you can't say why.

I don't start with what you want changed. I start with what's actually happening structurally — which compartment, which layer, which stage of the sequence you're in. Then we talk about what that means for now, and what it'll mean in five years, because a plan that only makes sense today isn't actually a plan.

Understanding the physiology isn't the boring part before we get to the good part. It is the good part. It's the difference between a treatment that makes sense and one that just adds something because that's what's on offer.

The unofficial DSA Run Club went LIVE at the Gold Coast Marathon this month.

smashed the marathon. smashed the 1/2 marathon and I got caught in the official photos on my phone doing phone consults.

There's a joke in there somewhere about the doctor who tells everyone else to switch off occasionally and then can't manage it herself for one Saturday morning to run a 10km race. I'll take it.

Massive effort from everyone who ran — genuinely proud watching this crew cross that line and repping my logo for 42.2km!! And thank you for making everyone look good!

I stopped negotiating on protocols a long time ago.

Not because I enjoy being the doctor who says no to a discount — I don't. But I've watched too many people spend money on a version of a treatment plan that was never going to get them the result they came in for. Two sessions when the skin needed four isn't a smaller win. It's not a win at all.

This is the part nobody wants to hear before they book: your skin doesn't know your budget. It doesn't do "almost." It responds to what it's given, consistently, over the time it actually takes to rebuild. If I can't get you there, I'll tell you before we start — not after you've spent the money.

That's the whole reason I diagnose before I treat. Not to upsell you into a bigger plan. To make sure the plan I give you is one that actually works.

"It's just pigmentation" is doing a lot of heavy lifting for a phrase that covers at least seven completely different conditions.

Post-inflammatory hyperpigmentation isn't melasma. Melasma isn't a solar lentigo. A solar lentigo isn't a naevus of Ota. They look similar enough in a mirror to get treated the same — and different enough under an actual diagnosis to need completely different plans. Some respond well to laser. Some get worse with laser. Some aren't pigmentation at all, and no amount of IPL is going to touch them.

This is the bit that doesn't show up in a five-minute consult: melasma that keeps coming back almost always means the hormonal or UV trigger was never addressed, not that the treatment failed. Same with PIH that resolves and then resurfaces — the inflammation source is still active somewhere. Treating the colour without treating the cause isn't a plan. It's a loop.

So before I switch anything on, I want to know what type this actually is, what's driving it, and what's happening in the skin underneath it. That's not extra. That's the treatment.

I started this clinic because there was a gap nobody was filling. Aesthetic medicine that was actually grounded in medicine. Not a treatment menu. Not a quick fix. Not a yes to everything because yes is easier than a proper consultation.

Bridging that gap was the whole point. Finding the patients who wanted that too — that was the hope.

June told me they exist. Every slot filled. And yes, the ads helped get the message in front of the right people. But ads only work when what you’re saying is true. The message landed because the clinic behind it is real.

That teamed with being recognised as a finalist in the CareSuper Business Awards. My mind is BLOWN!

This isn’t a brag. It’s a marker. A moment I want to remember when the floor hits different on a Wednesday and I’m questioning all of it.

It’s working. Keep going.

I see patients who’ve spent thousands across multiple clinics and still can’t tell me why their skin isn’t changing. When I ask what their plan is, they describe a list of treatments. That’s not a plan. A plan has a diagnosis at the top, a sequence underneath it, and a reason for every step. What most people have had is a menu — and a menu is just organised guessing.

A patient presents with pigmentation. Is it sun damage, melasma, a drug reaction, or something that needs a biopsy? Those are four different answers requiring four different approaches. An aesthetic clinic sees a treatment opportunity. A doctor sees a diagnostic question. That distinction matters more than most people realise.
