Last week, we had the pleasure of welcoming Community Health First and Eastern Health to our Ferntree Gully site to celebrate the expansion of the Care Pathways for Chronic Disease program.
This initiative helps people with chronic conditions manage their symptoms at home, with a dedicated health professional providing regular check-ins, education, and early care.
It builds on the success of our Health@Home Pathways program, bringing support to people like our customer John.
It was heartening to hear John share how this approach has changed his day-to-day life. With visits from his nurse Louise, he’s supported to monitor his health and understand his condition.
The expansion of the Care Pathways for Chronic Disease program is an exciting step forward for Victoria, supporting 4,500 vulnerable adults and easing pressure on hospitals across the state – so more people can get the care they need, sooner.
