🦷✨ Success with a free gingival graft starts long before the graft is placed.

One of the most overlooked factors is proper recipient bed preparation. The balance between vascular and avascular tissue plays a critical role in graft survival, revascularization, and the final clinical outcome.

A graft placed on a well-prepared, vascularized bed has the best chance of predictable healing. Too much avascular surface can compromise blood supply, while proper tissue management creates the environment needed for graft integration and long-term stability.

Periodontal surgery is not just about technique—it’s about understanding biology and respecting the principles that drive healing. 🩺🌱

Loose healing abutments and loose implant crowns are more than just a mechanical issue — they quickly become plaque traps.
Plaque accumulation around a loose suprastructure can lead to peri-implant inflammation and progressive bone loss if not detected early.
One of the most common mistakes is assuming that a crown or healing abutment is fully seated based on visual inspection alone.
The most reliable way to confirm that the suprastructure is fully seated on the implant is with a bitewing radiograph, not a periapical x-ray.
A small detail — but it can make a big difference in preventing peri-implant complications

Papilla between two implants vs. implant and natural tooth… not the same story.

When a dental implant is placed next to a natural tooth, the existing periodontal ligament and supracrestal tissue attachment of the tooth help support the interproximal papilla.

But between two implants?
We’re dealing with two peri-implant biologic widths — and that changes everything.

Unlike natural teeth, implants lack a periodontal ligament and have a different soft tissue attachment pattern. The vertical and horizontal dimensions of the peri-implant biologic width directly influence crestal bone levels… and bone is what supports papilla.

That’s why:
• Implant positioning (3D) is critical
• Inter-implant distance matters
• Platform switching and tissue thickness play a role
• And papilla reconstruction between two implants can be significantly more challenging

Understanding these biologic principles is key to predictable esthetic outcomes in the anterior zone.

Implant dentistry is not just about osseointegration — it’s about tissue architecture.
