In October, our sites celebrated a record: 11% of our patients successfully listed for transplant, in addition to over 30% actively undergoing workup for transplant. Behind those numbers are countless patient stories, and the tremendous efforts of our Transplant Coordinators working up each referral.

We do this work knowing that each patient listed for transplant has a chance for a life beyond dialysis. The smiles are never wider then when we see someone walk through our doors with their new, working kidney. 😊

Today, our Peterborough team got to share in one of those special moments with Joy, who after four years on dialysis returned to visit us with her new kidney! 🎉🎉

Joy came back to thank the team who had cared for her, especially our Peterborough Transplant Coordinator, Vanessa, for helping her through the process of getting listed and ultimately realizing this dream.

Congratulations, Joy! We couldn't be happier for you. ❤️

For years, hemodiafiltration (HDF) was one of the therapies offered across our sites. In the fall of 2023, that changed when we made HDF our primary dialysis therapy. You could say, we were CONVINCE-d 😋

Two years later, the results are in. After implementing HDF as our primary therapy we have seen sustained:

• 50% reduction in sub-optimal dialysis treatments (from 8% down to 4%)
• 31% reduction in patients with low hemoglobin levels
• Improved symptom management

Alongside these improvements, in 2025 we saw our lowest all-cause mortality rate in four years. 👏

Thank you to our Medical Directors, Dr. Nancy Barrese, Dr. Eliot Beaubien and Dr. Robert Ting for championing this therapy with us. We are also grateful to our nursing team, who continue to show adaptability and an unwavering commitment to the practice changes that enable our patients to receives the best possible outcomes in their kidney journey.

Innovation and quality improvement are central to our vision of modelling the future of healthcare. 💡

In 2025, DMC achieved two significant milestones. We became the first organization in Ontario to retrofit a Central Acid Delivery System, reducing plastic and dialysate-related waste by more than 80%, and the first site in Canada to perform an online dialysis treatment using the new 6008 machine. 🥇

We also strengthened partnerships that expanded access to care. Highlights included launching Markham's first pre-dialysis care program with Mackenzie Health, introducing onsite renal dietitian services with Scarborough Health Network, and hosting six vaccination clinics in partnership with local pharmacies. 🤝

Caring for patients with complex needs requires a focus on safety and clinical excellence. In 2025, DMC achieved zero critical incidents and maintained exceptionally low rates of catheter-related bacteremia and patient falls. We also helped reduce hospital utilization by providing 92 fluid management treatments and ensuring that 96.25% of patients received adequate or optimal dialysis. Notably, DMC did not require any patients to return to hospital for a higher level of care. 🏥

These outcomes were supported by our continued commitment to advanced therapies, including hemodiafiltration (HDF), transplant coordination initiatives, and treatments for kidney disease-related pruritus. At the same time, we expanded treatment capacity at our Markham and Pickering clinics to meet growing community needs. ✅

Better late than never 😊

Our 2025 Patient Experience Report provides greater insight into who we cared for and what was accomplished together with our patients and families.

In 2025, DMC supported 366 patients, delivered more than 30,000 treatments, welcomed 65 patients traveling from across Canada and abroad, and celebrated important milestones including 18 kidney transplants and successful transitions to home dialysis.

Most importantly, 99.2% of patients chose to continue receiving their care at DMC. We are so grateful to our patients and families for trusting us in their care. It is, and continues to be an honour.

The 2025 year was filled with many success stories; one though stood out from the rest.

Driven by our Markham and Pickering sites, DMC celebrated a 120% year over year increase in kidney transplants for our patients!

Thank you to all those involved in making this possible, especially our Markham and Pickering site Transplant Coordinators, Sara Bevilacqua and John Paul Sarte for helping our patients and families achieve this life changing milestone.

At Dialysis Management Clinics, we believe strongly that modelling the future of healthcare must include sustainability and climate conscious practices.

We are pleased to share that our Director, Ethan Holtzer will be helping to champion this very initiative provincially in a newly assigned role of ORN Provincial Administrative Champion for Sustainable Kidney Care. Alongside Dr. Seychelle Yohanna, he will be working collectively to provide oversight for the rollout of sustainable kidney care initiatives across Ontario’s 27 Regional Renal Programs.

A perfect illustration on how we can work together to light new ways in healthcare 💡

We’re proud to share that we have re-established our relationship with Dialysis Management Clinics (DMC), an affiliated dialysis provider, to help make ongoing dialysis care more accessible and convenient for patients living farther from our hospital sites.

Through this collaboration, eligible patients can receive dialysis services at a DMC location closer to home while continuing to remain under the care of our Nephrology Team. This means patients can spend less time travelling for treatment and more time focusing on their health, routines, and quality of life.

This work reflects the dedication of our dialysis and nephrology teams, whose collaboration, planning, and patient-centred approach helped move this relationship forward. We extend our appreciation to everyone involved in supporting this important step, including our physicians, leaders, allied health professionals, social workers, clerical teams, dietitians, pharmacy teams, and care partners.

📣📣Expansion Announcement! 🎈🎈

We are pleased to share that as of June 2026 we have expanded access to care at two of our clinic locations.

Credit to recent staffing increases, our Markham / North Toronto clinic can now accommodate four additional patients on our Monday, Wednesday, Friday, evening shift.

At our Pickering / Ajax clinic, a small scale expansion has enabled us to open a 20th dialysis station. This new station has created capacity for four additional patients across on our daytime shifts, while also supporting future growth on our evening shift.

We would like to extend our thanks to our dedicated team members, the Ontario Renal Network and our referring regional renal programs for their partnership and support in making this expanded capacity possible.

Together, we can continue to collectively meet the kidney care needs of the communities we serve and support.

DMC had the real pleasure to host members of our East Region’s Integrated Renal Program Council for a discussion on Sustainable Kidney Care, Hemodiafiltration (HDF) and the role DMC plays in the region’s kidney care system.

In attendance were members from two of our partner regional renal programs, Lakeridge Health and Peterborough Regional Health Centre.
