Taking or receiving a wrong medicine, or the wrong dose of a medicine, could harm your health.

MedError.ca, is part of the Canadian Medication Incident Reporting and Prevention System (CMIRPS), a Canadian initiative to prevent harmful medication incidents. The fact is, mistakes with medicines happen, but they are preventable. Mistakes with medicine are called medication incidents. We believe that consumers and health professionals can play active roles in preventing mistakes in health care.

When incidents are reported and reviewed, problems that could increase the chance of an error happening can often be found and corrected. The MedError.ca site provides a convenient way for consumers to report medication incidents and to get information about using medicines safely. At the Institute for Safe Medication Practices Canada, our job is to study the incidents reported by you and recommend ways to prevent them. The more incident reports we receive, the better we can work together to prevent harmful medication errors from happening in the future. MedError.ca is funded by Health Canada and is part of a Canadian initiative to prevent harmful medication incidents, the Canadian Medication Incident Reporting and Prevention System (CMIRPS). MedError.ca provides a better way for consumers to report medication incidents and to get information about using medication safely.

✋ Certain scenarios need the pharmacy to discontinue a previous prescription. This could be due to a new dose of a medication or a new medication that is replacing an old one.

ISMP Canada received a report from a patient who received the old dose of their medication after it had been changed.

Ask your prescriber to write the changes on the prescription for example, an increase⬆, a decrease⬇, a stop 🛑, etc. Check your prescription at pick up to ensure you have the ✅ right dose!

ISMP Canada is undertaking an independent evaluation to examine how ISMP Canada’s work has contributed to improving medication safety across Canada. The findings will support organizational learning, accountability, continuous improvement, and future planning.

Over the coming weeks, you will be invited to participate in a confidential interview, an online survey, or both. Participation is voluntary, and information collected will be kept confidential and reported only in aggregate.

there are many different potassium products available, but not all of them can treat the same conditions? ISMP Canada received reports related to dispensing the incorrect type of potassium to treat a specific condition.

If you have been prescribed potassium, it is important to:

✅ Ask your pharmacist if the potassium product you’re receiving is right for you.
✅ Speak with your pharmacist before switching between potassium products.

Easy dissolving powdered forms of medication can come in both adults and children's strength. Don’t get mixed up! Read the package carefully to make sure you’ve picked up the right dose.

ISMP Canada received a report from a consumer who accidentally gave their child the adult dose of a powdered medication because the boxes and the sachets looked similar.

Store them separately at home to prevent mix ups.

⚠Teva Canada Ltd. is recalling two lots of Teva-Pregabalin 150 mg capsules after routine testing identified that some capsules 💊 may be contaminated with the drug sertraline. Patients with a hypersensitivity or allergy to sertraline could experience side effects or have a serious allergic reaction, including an anaphylactic reaction, even at a low exposure.

🚑 If you experience a serious allergic reaction (anaphylaxis), stop taking the affected product and seek immediate medical help by calling 9-1-1. Otherwise, do not stop taking your pregabalin medication without first speaking with your healthcare professional or pharmacist, as stopping suddenly may cause withdrawal symptoms. Return the product to the pharmacy where it was purchased as soon as possible for a replacement or alternative supply, and for proper disposal. If you are unsure whether your product is recalled, check with your pharmacy.

⚠ Health Canada Public Advisory - One lot of New Roots Herbal Probiotic Intensity recalled due to incorrect “No Dairy” claim.

New Roots Herbal Inc. is recalling one lot of Probiotic Intensity because the outer box of the 30-count bottles incorrectly states that the product contains no dairy. The product contains bovine colostrum, which is a dairy ingredient. Bovine colostrum may pose serious health risks to people with a severe dairy allergy.
