We’re pleased to share an important update on how we are enhancing care and support for patients living with chronic conditions such as asthma, COPD, diabetes and cardiovascular disease.

💡 Introducing Abtrace – a smarter way to support your health
We are implementing a new digital system called Abtrace, designed to improve how we monitor, review and communicate with patients managing long-term conditions.

More personalised care – Abtrace helps our clinical team identify when reviews or check-ups are due, ensuring you receive timely and appropriate support.
Proactive health management – The system supports earlier intervention, helping to reduce complications and improve long-term health outcomes.
Better coordination of care – It enables a more streamlined approach to managing your condition across our team.
Convenient digital communication – You’ll be able to complete reviews and share information without always needing to attend the surgery in person.

📱 How we’ll keep in touch
As part of this new approach, patients will be contacted via text message and email. These communications may include:

Invitations to complete online reviews or questionnaires
Important health reminders
Requests for updates to help us better understand how you're feeling

These messages are designed to make it easier for you to stay engaged in your care and ensure we have the most up-to-date information to support you.

💜 Cervical Screening Week: 15-21 June 💜

Cervical screening is one of the best ways to help prevent cervical cancer, yet many people delay or miss their appointment. If your aged 25-64 and have a cervix, cervical screening can help detect changes early, before they become serious. The test is quick, simple and could save your life!

If you've received and invitation or think you may be due, please contact the surgery to book your appointment. Don't put it off - a few minutes could make such a big difference.
