If you would like the practice to make a Right to Choose referral for an Autism and/or ADHD assessment, please ensure you provide the following information:

✅ A completed Autism and/or ADHD questionnaire
✅ A personal statement from the patient (or parent/guardian if applicable) explaining why they feel an assessment is needed
✅ The name of your chosen Right to Choose provider
✅ An up-to-date email address and mobile telephone number

⚠️Please note: If any of the above information is missing, we will be unable to process the referral and the paperwork will be returned to you for completion.
