This delightful gentleman disliked his neck, and in particular the large fold of loose skin at the front. He said his granddaughter kept grabbing it and asked what it was, and he became self conscious about it.
Generally speaking, my preference is almost always to perform a deep plane neck lift, and this also involves the face being lifted to reposition soft tissues and remove the excess skin, through scars that are extremely well concealed. The deep plane face and neck lift is one of the most powerful, natural and long lasting procedures in facial aesthetic surgery.
In the case of this gentleman, he had a number of ongoing medical issues, which made a lengthy anaesthetic risky. So after careful counselling, we decided to proceed with an anterior direct neck lift, which is a simple 1 hour procedure under general anaesthetic. This kept it safe, and addressed his most pressing issue - the loose skin at the front. The trade-off however, is a vertical scar from the chin to the sternum, with a āZā in the middle (to break up the scar and allow it to sit in the contour of the neck). Scars are red and lumpy for a number of months, and require careful management with massage and silicone scar therapy, but it eventually fades extremely well.
I have done this procedure many times in suitable patients and every single patient has been satisfied with their outcome!
