Adult Wellbeing Services We are a range of services working together to reduce health inequalities and increase opportunities for individuals and communities across County Durham.

Offering advice and training to support behaviour change and improve health, wellbeing and learning opportunities

. Joining the Dots

Supporting people aged 18 and over to live well when they have received a cancer diagnosis, are going through cancer treatment or are living beyond their diagnosis. Joining the Dots Facilitators can also support immediate carers. Health Squad

Providing support for communities and individuals who have previously found it difficult to access primary health care for either cultural or personal reasons. Waiting Well

A programme for patients selected from surgery or cancer treatment waiting lists who are supported whilst they wait to ensure they are physically well in readiness for the surgery or treatment. Health Checks

Following up those identified as at risk of preventable illness following their NHS Health Check. The service signposts clients to health and wellbeing services for further advice and support. Cancer Awareness

A heart-warming comment from a client our ๐— ๐—ฎ๐—ฐ๐—บ๐—ถ๐—น๐—น๐—ฎ๐—ป ๐—๐—ผ๐—ถ๐—ป๐—ถ๐—ป๐—ด ๐˜๐—ต๐—ฒ ๐——๐—ผ๐˜๐˜€ team has been supporting recently. Our cancer support service is free to anyone who is:

- diagnosed with cancer
- a direct carer of those with a cancer diagnosis

๐Ÿ’™ Afternoon tea and wellbeing promotion with the residents of Magdalene Court, Seaham.

๐—ช๐—ฒ๐—น๐—น๐—ฏ๐—ฒ๐—ถ๐—ป๐—ด ๐—ณ๐—ผ๐—ฟ ๐—Ÿ๐—ถ๐—ณ๐—ฒ will be joining many other local organisations to mingle with the residents of the complex. We'll be sharing cuppas and helping them access the information and support for:

- finances
- isolation
- exercise
- activities
- bereavement

There will also be signposting to services if needed.

Date: Thursday 13 August, 3pm to 5pm

Learning new skills can improve your mental wellbeing by:

- boosting your self-confidence
- helping build a sense of purpose
- connecting you with others.

We deliver our sessions in community venues, online, and we can come to you.

One of our upcoming sessions is An Insight into Menopause. Recently we have had some feedback from a participant of a previous session:

โ€œ[A] fantastic, engaging speaker. I was worried at first when a male came on the screen, but I was pleasantly surprised at his knowledge both personally and professionally. He put me at ease straight away and I felt comfortable talking about such a personal issue.โ€

All our training and awareness sessions are funded by Durham County Council and free to attend for anyone living or working in County Durham.

๐Ÿ’™ Peaceful gardening for positive mental wellbeing.

๐—ช๐—ฒ๐—น๐—น๐—ฏ๐—ฒ๐—ถ๐—ป๐—ด ๐—ณ๐—ผ๐—ฟ ๐—Ÿ๐—ถ๐—ณ๐—ฒ is joining The Humble Garden CIC for a community allotment group. All are welcome โ€“ we are here to support mental health and to create a space to make friends.

Come down on these dates:

- Wednesday 9 September, 10:30am to midday
- Wednesday 16 September, 10:30am to midday
- Wednesday 30 September, 10:30am to midday
- Monday 5 October, 10:30am to midday
- Wednesday 14 October, 10:30am to midday

๐Ÿ’š ๐— ๐—ฎ๐—ฐ๐—บ๐—ถ๐—น๐—น๐—ฎ๐—ป ๐—๐—ผ๐—ถ๐—ป๐—ถ๐—ป๐—ด ๐˜๐—ต๐—ฒ ๐——๐—ผ๐˜๐˜€ has been supporting an elderly gentleman living with prostate cancer. To help with his mobility and personal care issues, his home had a stairlift and wet room installed. Both unfortunately were damaged and past their warranty.

Our team liaised with an Occupational Therapist, and a review of the issues and the gentlemanโ€™s needs was carried out. Because of this, he is now having a new wet room and stairlift installed at his home.

Weโ€™re so pleased to have been able to assist with this, as the gentleman said heโ€™d been trying to get by for some years. This will make a massive improvement to his home life and independence.
