Say Hello to our amazing Discharge to Assess Pathway 1 team! 🤝 We collaborate with the NHS to facilitate a smooth transition from hospital to home-based reablement in the Mendip area, supporting patients to achieve independence.

We're proud of the incredible success of Lifeline Homecare's Discharge to Assess – Pathway 1 Service, supporting patients across the Mendip area to leave hospital safely and continue their recovery in the comfort of their own homes.

Our dedicated team works closely with NHS partners to provide the care and support people need during those crucial first days after discharge. By helping patients regain confidence and independence at home, we're not only improving outcomes but also freeing up much-needed hospital beds for those who need them most.

Every successful discharge is a reminder of what can be achieved through partnership, compassion, and person-centred care.

A huge thank you to our amazing team for their professionalism, kindness, and commitment, and to our NHS colleagues for their continued collaboration.
