A 36-year-old woman with Fitzpatrick type I skin presents with an enlarging pigmented lesion on the posterior calf.

She reports previous sunburns and regular tanning bed use.

On examination, the lesion measures 1 cm and shows:

▪️ Asymmetry
▪️ Colour variegation
▪️ Irregular borders

🚨 New treatment options for uncomplicated urogenital gonorrhoea: the FDA has approved zoliflodacin (Nuzolvence) and gepotidacin (Blujepa) for patients aged ≥12 years.

The approvals provide additional oral treatment options at a time of increasing gonococcal antimicrobial resistance. Gepotidacin use is currently restricted to patients with few or no alternative treatment options because of limited safety data. 🦠💊

Chronic rhinosinusitis is more than a blocked nose:
🔹 Facial pressure
🔹 Postnasal drip
🔹 Congestion
🔹 General malaise

Perhaps surprisingly, quality of life scores can be worse than those of some other chronic conditions such as heart failure and asthma.

🚨 Heat stroke is not the same as heat exhaustion or dehydration. It’s a medical emergency.

As temperatures rise, keep your radar up for:
🧠 Altered mental state — confusion, agitation, delirium, ataxia
🌡️ Core temp >40°C
💓 Tachycardia + hypotension
🧪 Early organ dysfunction: jaundice, coagulopathy, rhabdomyolysis

High-risk groups aren’t just elderly patients:
⚠️ Drug use e.g., anticholinergics, diuretics, phenothiazines, tricyclic antidepressants
⚠️ Alcohol & stimulant use
⚠️ Athletes and people exercising intensely in hot, humid conditions
⚠️ Patients unable to care for themselves or access fluids appropriately

Key management priorities:
❄️ Rapid cooling + removal from heat source
💧 Fluid resuscitation with isotonic fluids
🩸 Monitor for rhabdo/DIC
⏱️ Time-to-cooling matters
