Living with more than one long-term health condition often means juggling several outpatient appointments, and it's not always easy when the care feels joined up one week and disconnected the next.

SEL ICB and the Health Innovation Network want to hear directly from you. If you're managing multiple long-term conditions in south east London — particularly heart, kidney, or metabolic conditions — please share your experience of outpatient care in this short survey. There's also the option to take part in a follow-up interview.

As the first point of contact for their 12,500 patients, you will play a crucial role in delivering excellent, patient-centered care. If you have great communication skills, a compassionate approach, and love working as part of a team, this is the perfect opportunity for you.

The Details:
💰 Starting Salary: £12.80 per hour (increasing after successful probation)
⏱️ Hours: Part-time (Up to 25 hours per week, Monday–Friday)
🗓️ Shift Requirements: Must be available Wednesdays, and a late shift until 6:30pm on Thursdays
📍 Location: Charlton, South East London (Blackheath & Charlton PCN)

You’ll join an incredible, collaborative team of GP partners, salaried doctors, nurses, and specialized PCN staff who truly value a supportive work environment.
