Being told you look tired when you’ve had a full night’s sleep can be frustrating.

But persistent tired-looking eyes are rarely explained by sleep alone. The appearance of the eye area can be influenced by pigmentation, visible vessels, tear-trough shadowing, fluid retention, skin quality, orbital fat, brow position and eyelid anatomy. Often, several of these factors are present at once.

This is why treating “dark circles” as one condition, or assuming that one treatment should work for everyone, can lead to disappointing results.

A cream cannot change a tear trough. Filler cannot remove orbital fat. Polynucleotides cannot correct significant structural hollowing. And laser should not be selected simply because the area looks darker. The starting point is understanding what is actually creating the appearance.

At Self London, treatment selection follows assessment of the skin and anatomy, with the aim of identifying what can realistically be improved, and when a procedure may not be proportionate to the expected benefit.

A closer look at one patient’s UltraClear treatment journey, including the days that follow treatment, when the skin is doing some of its most important work.

As we move into autumn and UV exposure naturally decreases, this is often the time of year when we can consider deeper, more intensive laser treatments, where appropriate.

For this patient, treatment planning also took her deeper skin tone into account. With energy-based treatments, the technology is only one part of the equation. The depth, settings and treatment approach need to be carefully considered according to the individual skin in front of us.

UltraClear can be tailored across different depths, allowing treatment to be matched to the concern being addressed, while taking into account the patient’s skin characteristics and expected recovery.

Healthy skin is a long-term investment.

Much of what determines how skin looks and behaves later in life is happening long before we see it in the mirror. Collagen begins to decline from our mid-twenties, while cumulative UV exposure contributes to changes in collagen, pigmentation and cellular DNA over time.

This is why skin health is better understood as maintenance rather than correction.

Daily broad-spectrum SPF remains one of the most effective interventions we have. Evidence-based topical treatments such as retinoids can support collagen remodelling, while appropriately selected laser and light-based treatments may have a role in improving the quality and resilience of photodamaged skin.

But treatment is only one part of the picture.

At Self London, we use VISIA imaging to establish an objective baseline, allowing us to assess features that aren’t always visible to the naked eye and monitor how the skin changes over time.

For us, this number is not simply a milestone. It represents experience.

In 2024, Self London became the first consultant dermatologist-led clinic in Europe to introduce AviClear. Since then, we have completed more than 1,000 treatments.

That means more than 1,000 opportunities to understand how different patients respond to treatment, to refine protocols, to manage the practicalities of a course of treatment and to build the clinical judgement that comes with seeing patients over time.

AviClear is a significant part of our approach to acne, but technology is only one part of the treatment experience. How it is delivered, how patients are supported throughout their course and the experience of the clinician behind the device all matter.

A new development in medical weight management has arrived in the UK.

The Wegovy tablet is the first GLP-1 tablet licensed specifically for weight management, offering an alternative to injectable treatment for appropriate patients.

Containing semaglutide, the same active ingredient as injectable Wegovy, it works by reducing appetite and increasing feelings of fullness. But its role should not be reduced to simply being another way to lose weight.
For appropriate patients, medical weight management can play an important role in improving metabolic health and supporting long-term health outcomes.

The Wegovy tablet, injectable Wegovy and Mounjaro each have different mechanisms, evidence and practical considerations. There is no single treatment that is right for everyone.

Sun damage doesn’t always look like sunburn.

Over time, cumulative UV exposure can show up in more subtle ways like changes in pigmentation, uneven skin tone, redness, texture and the gradual breakdown of collagen and elastin. These changes are often referred to as photoageing, and they can continue to develop long after the initial exposure has taken place.

In a recent feature with Who What Wear UK, Dr Mahto discusses some of the most common signs of sun damage and what can be done to address them.
