How did one major trial change the treatment approach for high-risk early triple-negative breast cancer (TNBC)?🤷‍♀️

The KEYNOTE-522 trial looked at adding the immunotherapy drug pembrolizumab to chemotherapy for people with previously untreated, high-risk stage II–III TNBC💊

🔹 Before surgery: Pembrolizumab + chemotherapy

🔹 After surgery: Pembrolizumab continued alone for up to 9 cycles

🔹 PD-L1 status: Patients were included regardless of PD-L1 expression

At 5 years:

📈 86.6% of people receiving pembrolizumab + chemotherapy were alive, compared with 81.7% receiving chemotherapy alone.

The takeaway? Adding pembrolizumab improved 5-year overall survival in KEYNOTE-522, helping establish pembrolizumab + chemotherapy as an important treatment approach for high-risk early TNBC💜

A blood test may now help guide a treatment change before metastatic HR+/HER2− breast cancer is visibly progressing on a scan 🧬

ESR1 mutations can develop during treatment with an aromatase inhibitor and make the cancer resistant to it.

In SERENA-6, switching to camizestrant when an ESR1 mutation was found in ctDNA delayed progression to 16.0 months vs 9.2 months when treatment was continued 💊

The FDA has now granted accelerated approval to camizestrant with a CDK4/6 inhibitor for this approach ✅

Trodelvy (sacituzumab govitecan) is now FDA-approved as a first-line treatment for some people with metastatic triple-negative breast cancer (TNBC)✅

💊 This applies to people with unresectable locally advanced or metastatic TNBC who are not candidates for PD-1/PD-L1 immunotherapy.

🔬 The ASCENT-03 trial compared Trodelvy with standard chemotherapy in people who had not previously received systemic treatment for advanced disease.

📊 Results showed:

✨ People receiving Trodelvy went a median 9.7 months without their cancer growing or spreading

✨ This compared with 6.9 months for those receiving chemotherapy

⏳ Overall survival data are still being followed, so we don’t yet know the full impact on survival.

🚨Trodelvy received FDA approval for this first-line use in June 2026.

New research suggests some breast cancer survivors whose heart function was affected by HER2-targeted treatment may not need to stay on heart medication for life 💜

In a small trial of 90 women whose heart function had recovered:

🔹 46 gradually stopped their heart medication, while 44 continued

🔹 After 1 year, 98% of those who stopped had no decline in heart function

🔹 1 woman experienced a decline, which improved after restarting medication

🔹 No heart failure symptoms or hospital admissions were reported

The study was small and follow-up was only 1 year, so more research is still needed. But the findings suggest that for some people, lifelong heart medication may not always be necessary once heart function has recovered 🙌

Any changes to heart medication should only be made with medical supervision and ongoing monitoring ‼️

For people with a PIK3CA mutation, VIKTORIA-1 also showed promising results, and an application has now been submitted to the FDA to expand the approval to this group 💡
