How They Support You:
• One-to-one coaching
• Advice on benefits & housing
• Workplace conversations

For Employers:
• Advice on Reasonable Adjustments

Who can join?
• Aged 18–64
• Have a current FIT Note and at least one previous in the last 6 months
• Self-referral or GP referral available

How They Support You:
• Personalised support for your health and work
• Help with confidence, coping, and isolation
• Access to groups and funded therapies
• Advice on workplace adjustments
• Support with your fitness
• Funding available to overcome issues such as travel
