Vascular Parkinsonism
Vascular Parkinsonism is a neurological disorder that causes symptoms similar to Parkinson’s disease but is caused by problems with the blood supply to the brain rather than the gradual loss of dopamine-producing nerve cells. It usually develops after multiple small strokes or from long-term damage to the brain’s small blood vessels. Although it shares many features with Parkinson’s disease, vascular Parkinsonism has different causes, symptoms, diagnosis, and treatment. Understanding these differences is important because early recognition and management can help improve a person’s quality of life and reduce the risk of further strokes.
Causes
Vascular Parkinsonism occurs when blood vessels supplying the brain become damaged or blocked. This damage may result from a series of small strokes, known as lacunar infarcts, or from chronic small vessel disease. These injuries affect areas of the brain responsible for controlling movement, leading to Parkinson-like symptoms.
Several factors increase the risk of developing vascular Parkinsonism, including high blood pressure, diabetes, high cholesterol, smoking, obesity, heart disease, and previous strokes. As people age, the risk of both stroke and vascular Parkinsonism also increases.
Symptoms
The symptoms of vascular Parkinsonism often differ from those of Parkinson’s disease. The condition mainly affects the lower part of the body, leading to difficulties with walking and balance. Common symptoms include:

Unlike Parkinson’s disease, resting tremor is less common in vascular Parkinsonism.
Diagnosis
There is no single test to diagnose vascular Parkinsonism. Doctors diagnose the condition by reviewing the patient’s medical history, performing a neurological examination, and using brain imaging such as magnetic resonance imaging MRI or computed tomography CT scans. Brain scans often show evidence of small strokes or widespread small vessel disease, supporting the diagnosis.
Doctors also consider whether symptoms began after a stroke and whether the patient has risk factors for vascular disease.
Treatment
There is currently no cure for vascular Parkinsonism. Treatment focuses on controlling the underlying vascular disease and improving mobility.
Treatment may include:
* Medications to control high blood pressure, diabetes, and high cholesterol
* Blood-thinning medications if appropriate to reduce the risk of future strokes
* Physiotherapy to improve strength, balance, and walking
* Occupational therapy to help with daily activities
* Speech and language therapy if speech or swallowing difficulties develop
* A trial of Parkinson’s medication, such as levodopa, although the response is often less effective than in Parkinson’s disease
Lifestyle changes such as regular exercise, eating a healthy diet, stopping smoking, limiting alcohol intake, and maintaining a healthy weight are also important.
Complications
If left untreated, vascular Parkinsonism can lead to repeated falls, fractures, loss of independence, reduced mobility, and an increased risk of further strokes. Some individuals may also develop problems with memory, mood, or bladder control.
