NHS England are writing to selected patients to check that we hold the correct residential address for you as eligibility to remain as a patient of the practice.

Please DO NOT ignore this letter 📩. This is NOT a scam and we will support you with this if you need it.

You are asked to confirm your details using specific details in that letter. If you do not respond with 30 days, they will remove you from our patient list, and you will no longer be registered here at The Thornton Practice. ☹️

We currently have over 70 patients that have been submitted to be deducted from our register by NHS England! 😣

We have no control over this process and this will delay any NHS care you receive from us and may cause issue with hospital appointments or letters if you ignore it. 😳

Please check with elderly relatives, friends and your family members to ensure they respond if they receive a letter. ❤️‍🩹

Dear Patient,

Your GP practice is one of a group of local practices that are coming together to form a federation of local practices in Blackpool, Fylde & Wyre.

This partnership will allow our practices to work together more effectively — sharing resources, offering more services locally and having a stronger voice in decisions about healthcare in our community.

It will not change your registration with your current GP practice, and your relationship with your own practice will continue as it is today.

Before we decide what this federation should focus on and how it should work, we want to hear from the people who matter most: our patients.

This short survey asks for your honest views about what matters to you about your GP care — what you value most, what you would never want to change and what could be better.

There are no right or wrong answers. Every response will be read and will help shape what this partnership stands for and what it commits to.

We would just like to thank you all for your patience with our staff today as they embarked on this new way of working through total triage.

As expected there have been some teething issues, one of which being the access through our NHS app! Unfortunately, this didn't go live like we thought it would however, you can still submit via our website link for now until our NHS app can be linked correctly to the new triage system.

We understand that some patients may feel worried about the move to Total Triage, particularly those who do not use the internet or are less confident with online services.

We would like to reassure all patients that nobody will be disadvantaged by this change.

Our phone lines will continue to be open from 8:00am to 6:30pm, and our reception team will still be available to help patients access care. If you are unable to complete the online form yourself, a member of our team can complete it on your behalf over the telephone.

The new system is designed to improve access for everyone by spreading demand across multiple routes, rather than relying solely on the traditional 8am telephone rush. Patients can contact us online, through the NHS App (once this is linked), or by phone, allowing requests to be submitted in a way that works best for them.

By clinically triaging requests first, we can ensure patients are directed to the right clinician or service in the most appropriate timeframe, helping us provide safer and more efficient care for all.

Thank you for your continued patience and understanding as we introduce this new way of working to create a better future fom our patients.

📢 Important Patient Update – Total Triage Goes Live TOMORROW (14/05/2026)

From tomorrow, our practice will be moving to a Total Triage model.

What this means: All requests for appointments will first be clinically triaged by a GP before an appointment or advice is offered. This helps ensure patients receive the most appropriate care, in the right timeframe, from the right healthcare professional.

🕢 Requests can be submitted from 7:30am each morning via:
• The NHS App
• The link on our practice website
• By calling the practice and a member of the team can submit the form on your behalf

We kindly ask patients to provide as much information as possible when submitting their request, as this helps our clinical team assess your needs safely and effectively.

From Thursday 14th May, we will be moving to a Total Triage Model.

👉 What does this mean?
All requests for appointments will be clinically triaged before an appointment is offered.

📝 How it works:

You will need to complete a triage questionnaire about your symptoms
This will be reviewed by a GP, who will then provide the most appropriate advice or appointment
The form can be submitted via the NHS App or through a link on our website
📞 No internet access?
If you’re unable to use online services, please call the surgery and a call handler will complete the form on your behalf.

📄 Administrative requests
You can also submit admin requests online, including sick notes, and a member of our admin team will get back to you.
