Extended use of Nexplanon from 3 to 5 years

MHRA has approved a change to the maximum licensed duration of Nexplanon use from 3 to 5 years. This follows similar approval by the FDA in USA earlier this year and is supported by the College of Sexual& Reproductive Health.

What it means for patients

As of now, patients attending for a routine change of their Nexplanon should be advised that this can now be delayed until 5 years. If they are in their 4th or 5th year of use and requesting early change due to problems such as altered bleeding pattern, then assuming other possible causes have been excluded ( e.g., STI, pregnancy, cervical pathology, concurrent medications affecting absorption etc) early change may be considered on an individual patient basis.

However, there is no good evidence that changing the implant early will necessarily improve problematic bleeding. Irregular bleeding is very common with the implant and bleeding patterns are often unpredictable and can change during the period of use. Other strategies to improve bleeding e.g., addition of COC for 3/12 or short 5 day course of an NSAID (if medically eligible), should also be considered if the patient wishes to continue the method.

After five amazing years, the time has come for me to say goodbye to my role as a Treatment Room Nurse at West Calder Medical Practice.

This has been a hard decision to make because this practice has been so much more than just a workplace. It has been a place where I’ve grown as a nurse, built lifelong friendships, and had the privilege of caring for the most wonderful patients.

To every single patient who has trusted me with their care over the years, thank you. Thank you for allowing me to be part of your lives, for your kindness, your patience, your laughter, and for the many conversations that made my days so special. Whether you came to see me for a dressing, blood tests, injections, ear syringing, or simply stopped for a chat, you’ve all left a lasting impression on me. It has truly been an honour and a privilege to care for you, and I will always be grateful for the trust you placed in me.

To my incredible colleagues, thank you for your friendship, support, encouragement and for making every day enjoyable, even on the busiest days. I feel incredibly fortunate to have worked alongside such a dedicated and compassionate team, and I will always value the memories and experiences we have shared.

While I’m excited to begin a new chapter in my nursing career, I will always look back on my time at West Calder Medical Practice with pride and gratitude. This practice, its staff and its patients will always hold a special place in my heart.

It has been a genuine privilege to care for such a wonderful community, and I will always look back on this chapter with great fondness and appreciation.

A message from Caroline.

I would like to thank all the GPs, staff, past colleagues, friends, and patients for the incredible kindness shown to me on my retirement. I truly thought I would just slip quietly out the door, but your generosity has been overwhelming. The flowers, gifts, cards filled with such thoughtful words, and the lovely comments on Facebook have meant more than I can say.

I have absolutely loved my job in reception at the surgery over the past 26 years and dealing with all the patients, not only were they patients but good friends too.

I wish everyone the very best for the future, and thank you again for all the kindness you have shown me.

Update on Wegovy and NHS Lothian Referrals

Following the recent announcement confirming that Wegovy has now been approved for use within NHS Scotland, we know many patients are looking for more information.

NHS Lothian also announced yesterday that all weight‑management referrals are currently paused while the new pathway is being finalised.

Please remember that Wegovy is a prescription‑only medicine and should only be used under clinical supervision. We’ll share updates as soon as NHS Lothian confirms the revised pathway.

Please note the Weight Management Service is not able to accept referrals for Obesity Management Medicines at this time.
