This is your sign to create one TODAY. 📁

When seconds count, paramedics and ER staff need information fast. This simple packet could save your loved one's life.

Here's what to include:

📋 PERSONAL INFO
• Full name, address & date of birth
• Photo ID (driver's license or state ID)

💊 MEDICAL INFORMATION
• Complete medication list with dosages & purpose
• All allergies (medications, food, environmental)
• List of chronic conditions & diagnoses
• Recent lab results (EKG, CBC, kidney/liver function)

👨‍⚕️ CONTACTS
• Names & numbers for all doctors & specialists
• At least 3 emergency family contacts
• Note who holds Power of Attorney

🪪 INSURANCE & FINANCIAL
• Insurance cards (front & back) — Medicare, Medigap, Part D
• Copy of credit/ID cards
• Small amount of cash

⚖️ LEGAL DOCUMENTS
• Durable Power of Attorney
• Health Care Proxy / Advance Directive (with HIPAA authorization)
• Living will & life insurance info

📍 WHERE TO KEEP IT:
Put it in a brightly colored binder near the front door or on the refrigerator so emergency responders can find it immediately. Give a copy to all family members and caregivers and UPDATE it whenever medications or doctors change.

Medicare requires hospitals to provide written notice of discharge plans. If that plan doesn’t feel right, you can request a formal review.

Before your aging parent leaves the hospital, there’s something you should know.
When a discharge is being planned, it can feel like the decision is already made. Home or rehab - and you’ve got to move quickly.

Once a discharge happens you’re often left trying to piece a plan together to fill care gaps after the fact, which can be challenging to say the least.

But Medicare requires hospitals to give a specific written notice that you’ll see with the title ‘Important Message from Medicare’ that explains your rights around discharge.

That notice outlines when and how you can ask for a review if the discharge plan doesn’t feel right.

Asking for a review often slows things down and allows you a bit more time to be sure the plan fits your parent’s needs.

Today we celebrate one of the most powerful things older adults can do for their health: keep moving! 💪

At Premier Care Management, we know that staying active is about so much more than the gym. Here are 3 simple tips to help you or a loved one get started:

🌿 Tip #1 — Start with a Walk. Walking is a perfect way to start exercising it requires no special equipment and can be done almost anywhere. Even a short stroll around the neighborhood counts!
🌿 Tip #2 — Don't Skip Balance & Flexibility. Falls are the leading cause of injury and death among people aged 65 and over, incorporating activities like tai chi, yoga, or simple daily stretching can help maintain your balance and flexibility. Chair yoga is a great, low-impact place to start!
🌿 Tip #3 — Make It Fun & Social. A big part of sticking to any exercise routine is doing activities you enjoy, variety helps keep burnout from setting in, so don't be afraid to change things up! A dance class, a walk with a friend, or even gardening all count.

Sometimes families come to us after they've already tried everything. They're exhausted. And it feels like nothing is going to move. That's exactly where Janis met this family and she got to work.

The rehab stay went well. But then came discharge day. And that's when things fell apart. It happens more than most people realize.

The plan sounds good on paper, but nobody accounted for what home actually looks like. The equipment isn't there yet. The follow-up appointment is weeks out. And the family is left figuring it out in real time, exhausted and scared.

Transitions between care settings are one of the riskiest moments in an older adult's journey. And having someone in your corner during that time makes all the difference.
At Premier Care Management, our care managers are present during rehab and skilled nursing stays not just at the end, but throughout. Your friend is caring for an aging parent. They won't ask for help, but here's how you can show up for them.
Check in on them, not just their parent, ask "how are you doing?" and actually wait for the answer.

1. Offer something specific instead of "let me know if you need anything" - that phrase puts the burden back on them.
2. Invite them to do something normal, caregivers often disappear from social life without anyone noticing.
3. Be the person they can say the hard stuff to, sometimes they just need someone who won't judge them for feeling exhausted or overwhelmed.
4. Recommend they connect with a geriatric care professional or another aging professional to find resources for you.

The people doing this work quietly need us to show up loudly. 💙
