Hospice care isn’t a place. It’s a type of care. 💙

One of the most common misconceptions about hospice is that a patient has to move into a facility to receive services. In reality, hospice is designed to meet patients where they are and where they feel most comfortable.

That may be their own home, an assisted living community, a nursing facility, or another place they call home.

Our interdisciplinary team works alongside patients, families, caregivers, and facility staff to provide symptom management, education, emotional and spiritual support, personal care, and guidance throughout the hospice journey.

The goal is simple: bring comfort and support to the patient, wherever they are.

Building relationships, sharing meaningful conversations, and supporting our senior community is what we’re all about. Thank you to everyone who welcomed us—we’re grateful for the opportunity to connect and continue serving our community.

Hospice is more than nursing care - it’s a team approach. 💙

Every patient at Luminos Hospice is supported by an Interdisciplinary Team (IDT) made up of professionals who work together to meet physical, emotional, spiritual, and practical needs.

From nurses and physicians to social workers, chaplains, hospice aides, volunteers, and bereavement specialists, each member plays an important role in helping patients and families navigate their journey with comfort, dignity, and compassion.

Because no one should face this journey alone.

Today, we celebrate the fathers alongside us, the fathers we serve, and the fathers whose love, guidance, humor, strength, and sacrifice continue to shape the lives of those around them.
