In a landmark move on May 4, 2026, several federal agencies—including SAMHSA, CMS, HRSA, and ACF—issued a joint letter emphasizing that mental health care must be more than just a prescription. The Department of Health and Human Services (HHS) is now formally advocating for "meaningful access" to evidence-based non-pharmacological interventions.

At Synaptic PSYCH, we have always believed that true healing involves the whole person. We are thrilled that the federal government validated the very treatments we offer every day at all our offices, grounded in the principles of shared decision-making and patient autonomy.

The Power of Non-Pharmacological Care

The latest clinical evidence and federal guidelines suggest that for many conditions, lifestyle and behavioral interventions can be as vital as traditional medicine. We are proud to offer a robust continuum of care that includes:

Psychotherapy and Behavioral Approaches: Recognized as essential first-line treatments for conditions like depression.
Physical Activity and Exercise: Recent research syntheses confirm that exercise can effectively reduce depressive symptoms.
Nutritional and Dietary Strategies: Evidence supports dietary improvement interventions as a way to improve symptoms in various populations.
Sleep-Focused Treatments and Social Connection: These are now identified as key evidence-based interventions for a comprehensive treatment plan.

A Note of Caution: Evaluating ONE-DAY TMS for Depression
Transcranial Magnetic Stimulation (TMS)
Optimizing your mental health is a journey often paved with new technologies and promising protocols. One such advertised method, Optimized, Neuroplasticity-Enhanced techniques in Depression (ONE-DAY TMS), has garnered attention for its compressed timeline and combined approach. However, the Clinical Transcranial Magnetic Stimulation Society (CTMSS), dedicated to evidence-based practice, has recently issued a statement advising caution regarding current claims about its efficacy.

What is ONE-DAY TMS?

ONE-DAY is a specific accelerated Transcranial Magnetic Stimulation (TMS) protocol. It involves a single-day treatment regimen consisting of twenty sessions of 600-pulses intermittent theta burst stimulation. In addition to the stimulation, patients are administered specific oral medications—125 mg of d-cycloserine and 20 mg of lisdexamphetamine—in the morning prior to the treatment sessions.

While single-day and medication-assisted TMS protocols hold significant potential for establishing clinical efficacy in the future, it is crucial to understand the state of the evidence right now.

VNS Therapy: A “Pacemaker for the Brain”
VNS
When standard medications and therapy fail to lift the weight of Treatment-Resistant Depression (TRD), medical technology offers a different path. Vagus Nerve Stimulation (VNS) Therapy uses a small, FDA-approved device—often called a “pacemaker for the brain”—to target the body’s natural neural highways.

VNS Therapy: A "Pacemaker for the Brain"How the Device Works
Unlike daily pills, VNS is an implantable system. During a short outpatient procedure, a pulse generator is placed under the skin of the chest. A thin, flexible wire then connects this generator to the left vagus nerve in the neck.

Pulse Delivery: The device sends regular, mild electrical pulses to the brain via the vagus nerve.
Mood Regulation: These signals reach areas of the brain responsible for mood, such as the limbic system, helping to “reset” dysfunctional neural circuits.
What to Expect
VNS is not an overnight fix. While some notice changes early on, the most significant benefits typically accumulate over 6 to 12 months. The newest data, shows that patients continue to improve into the second year after implantation. For many, this long-term approach provides a sustained reduction in symptoms and a significant improvement in quality of life when other options have been exhausted.

A New Horizon in Depression Treatment: Flow Neuroscience tDCS Device Receives FDA Clearance!
Exciting news for those battling Major Depressive Disorder (MDD)! The Flow Neuroscience tDCS device has just received FDA clearance, marking a significant step forward in accessible and innovative depression treatment. This non-invasive, at-home device offers a promising new option, especially for individuals seeking alternatives or complements to traditional therapies.

What is tDCS and How Does Flow Work?
Transcranial direct current stimulation (tDCS) is a gentle, brain-stimulating technique. The Flow device works by delivering a mild electrical current to specific areas of the brain associated with depression. This subtle stimulation helps to rebalance neural activity, aiming to improve mood and cognitive function.

Beyond the Diagnosis: The Future of the DSM is Getting More Personal

In the world of mental health, a diagnosis has long been a binary: you either meet the criteria for a disorder, or you don’t. But as any clinician (or patient) knows, human experience rarely fits into a neat little box.

A recent article by Mark Moran in Psychiatric News highlights the work of the APA’s Future of DSM Strategic Committee, a group of experts working to evolve the “Psychiatrist’s Bible” into a more holistic, “living document.”

Here is a breakdown of the four major shifts coming to the future of psychiatric diagnosis:

Is There a Difference Between Ketamine and Esketamine (Spravato) for Depression?
The discovery of ketamine’s and esketamine’s rapid antidepressant effects has been called the greatest breakthrough for depression research in the last 60 years. There is a big debate about whether there is a significant clinical difference between the two forms: racemic ketamine (used off-label for depression) in an intravenous (IV) or intramuscular (IM) and esketamine (an FDA-approved nasal spray, Spravato).

As a physician practicing in Tennessee, I frequently work with patients who are becoming eligible for Medicare. My goal is to help them understand this federally subsidized health insurance system, so they can make informed decisions and feel confident about their coverage.

What Tennessee Patients Need to Know
Medicare education for Tennessee patients

This federally funded program provides free, unbiased counseling and assistance for Medicare-eligible individuals, their families, and caregivers. Importantly, TN-SHIP counselors do not promote any insurance agency, and all inquiries remain confidential.

How to contact TN-SHIP:
