Central Wyoming Hospice & Transitions

Central Wyoming Hospice & Transitions Our Mission: To companion the end of life journey with skill and compassion. We focus on pain and symptom management to improve quality of life.

We can’t add days to your life, but we can add life to your days.

06/09/2026

For our physician partners: Early hospice referrals change outcomes.

When patients enter hospice care earlier, they experience better symptom management, fewer emergency visits, and more time for what matters most. Research shows early enrollment improves both quality of life and family satisfaction with care.

Signs it may be time to consider a hospice consultation:

• Declining function despite optimal treatment
• Frequent hospitalizations or ER visits
• Progressive weight loss or decreased oral intake
• Patient or family asking about comfort-focused care
• You find yourself wondering, "Would hospice help here?"

We're here as a resource, not a replacement. Our team works alongside you to provide:

💜 Expert pain and symptom management
📞 24/7 on-call nursing support
🤝 Care coordination that complements your treatment plan
🌿 Family guidance and caregiver support

A consultation doesn't require commitment. It's simply a conversation about whether hospice could help your patient and their family.

Let's talk. 307.577.4832

Grief doesn't follow a timeline. Neither does the need to honor the people who shaped our lives.

If you've been looking for a way to memorialize someone you love, or celebrate someone who made a difference, our memorial pavers offer a lasting, tangible tribute.

Each paver is engraved with the wording of your choice and placed on the grounds surrounding Central Wyoming Hospice. It becomes part of a garden of remembrance, a quiet space where names and legacies are held with care.

And every paver helps ensure that other families receive the compassionate end-of-life care and grief support they need—just like you or your loved one may have received.

Your loss matters. Their life matters. And this is one way to make sure both are remembered.

Healing looks different for kids.🐴

Sometimes it looks like laughter in the middle of hard feelings.
Sometimes it looks like quiet moments, new friendships, or simply knowing they are not the only one.

Kids’ Grief Camp creates space for all of that.

Through connection, activities, and time outdoors, children and teens can begin to process loss in a way that feels natural to them, supported by people who understand.

Open to youth ages 6 to 16 who have experienced the loss of a family member or friend.

Two summer sessions available. Space is limited.

This Memorial Day, we remember and honor those who served.

At Central Wyoming Hospice, we’re privileged to care for veterans in their final chapter. Through our We Honor Veterans program, we provide specialized support that recognizes military service and helps veterans find peace.

Every veteran in our care receives a Pinning Ceremony, a moment to honor their service, their sacrifice, and their story. It’s one of the most meaningful things we do.

To the veterans and military families in our community: thank you. Your service will never be forgotten.

💙 Celebrating Professional Milestones 💙

We are proud to celebrate our amazing graduates, Lexus, Dakota, and Jennifer! Lexus graduated from Casper College’s Adult Education program, Dakota earned his Associate of Science degree and will next enter the nursing program in Douglas, Wyoming, and Jennifer earned her Master of Social Work degree! What a big year for our staff!

These accomplishments reflect their dedication, perseverance, and commitment to growth. We are incredibly thankful to have them as part of our team and can’t wait to see all they continue to achieve!

Congratulations!! 🎉👏

It is hard to put into words what hospice nurses truly do each day.
Because so much of it cannot be measured.

They manage pain and symptoms. They coordinate care.
But just as much of their time is spent in the quiet, human moments.

Listening to stories that matter.
Helping families understand what is happening and what to expect.
Sitting in silence when there are no words.
Answering the same questions again and again, with patience and care.

They notice the small things. A good night’s sleep. A favorite meal. A moment of laughter.

Our nurses do not solely provide medical care. They bring presence, patience, and a sense of calm when families need it most.

That is hospice nursing.
Clinical expertise, grounded in compassion.
