What Every Man Should Know About Testosterone Replacement

The most commonly used form of testosterone replacement. This is a plant-based hormone, meaning it closely resembles the body's naturally produced hormone rather than a synthetic alternative. Patient of the Month β€” Father's Day Edition

This month, we are proud to celebrate Oscar Andrade.

Oscar came to Vitality MD one year ago with three clear goals: lose weight, sleep better, and regain his energy. Behind those goals was a man carrying more than he realized β€” a fatty liver with active inflammation, chronic insomnia that had gone unaddressed for years, and the kind of fatigue that accumulates quietly when the body is no longer functioning as it should.

After a thorough evaluation, we identified that his insomnia was significantly driven by low testosterone. Once hormone replacement therapy was initiated, his sleep began to improve and with better sleep came better metabolic function. His first 12 pounds came off through that hormonal and restorative foundation alone.

When progress plateaued, we made a strategic clinical decision to add Tirzepatide, starting at 2.5 mg and titrating to 5 mg. To date, Oscar has lost 19 pounds of body fat. That is not simply a number on a scale. Body fat is metabolically active and pro-inflammatory, and its reduction has had a direct clinical impact. His liver inflammation has resolved completely and his energy has returned in a way he had not experienced in years.

Testosterone replacement therapy is one of the most discussed and most misrepresented treatments in men's health today.

Social media has framed TRT as a broad solution for fatigue, low motivation, and the general effects of aging. The clinical evidence tells a more precise story.

TRT is a medical therapy indicated for men with documented hypogonadism, defined by both consistent symptoms and repeatedly confirmed low testosterone levels. It is not an anti-aging intervention, and it is not appropriate for every man who feels fatigued or suboptimal.

What the evidence supports is meaningful. Randomized controlled trials, including the Testosterone Trials published in the New England Journal of Medicine in 2016, demonstrate that TRT in appropriately selected men improves libido, sexual satisfaction, lean muscle mass, bone mineral density, and visceral fat. The T4DM trial found that testosterone therapy combined with structured lifestyle intervention significantly reduced progression to type 2 diabetes compared to placebo.

What the evidence does not support is equally important. TRT does not reliably improve cognition, mood, or physical performance in men with normal testosterone levels. Treating symptoms without confirmed deficiency is not evidence-based practice.

TRT requires careful patient selection, individualized dosing, and consistent monitoring including cardiovascular status, blood counts, prostate health, and metabolic markers. The clinical objective is not to maximize testosterone. It is to restore physiologic balance safely within a framework of ongoing medical supervision.
